Good Shepherd Hospice & Catholic Health Home Care

Good Shepherd Hospice & Catholic Health Home Care GSH & CHHC are members of Catholic Health, providing expert medical, emotional and spiritual care.

Good Shepherd Hospice (GSH) is a non-profit organization that was created in 2001 with the merger of Good Samaritan, St. Charles and Mercy Hospices and represents a combined thirty years of experience serving the community. Our mission is to expand and enhance healthcare at the end-of-life for all individuals and their families living throughout Long Island, and to educate the community about hospice services. Good Shepherd Hospice helps patients and their families live each day fully, focusing on quality of life, control of symptoms and the things that matter to them the most. Catholic Health Home Care (CHHC) is a non-profit, non-denominational, home care agency certified by the Joint Commission. Catholic Home Care services more than 2,100 patients in their residences through the acute care agency and over 200 long term patients each day. The Home Care team consists of more than 550 dedicated professionals, para-professionals, and support staff to ensure that patients continue to have their home care needs met twenty-four hours a day, seven days a week. Specialty services include skilled nursing, physical therapy, occupational therapy, speech language pathology, nutritional counseling, medical social work, and home health aides. We have program specialties with highly trained staff to include pastoral care, behavioral health, infusion therapy, wound management, palliative care, and telehealth.

This November, during , we’re proud to celebrate every member of our incredible team and the community members we are honored to serve each day.

Meet Melinda St. Simon, Director of Clinical Education, who shared:

“I joined Catholic Health Home Care and Good Shepherd Hospice to make a meaningful impact where care truly meets compassion—within the homes of our patients and their families. Each day, I’m inspired by the opportunity to support our dedicated staff through education and mentorship, helping them feel confident and equipped to provide exceptional care during life’s most tender moments. I’m especially proud of our RN Residency Program, which has achieved a 90% retention rate and 100% confidence among nurses entering the field of home health and hospice. Watching its success lead to the creation of new LPN and PT residency programs has been incredibly rewarding, as these initiatives continue to strengthen our workforce and shape the future of compassionate home care.”

Thank you, Melinda, for your leadership, compassion, and dedication to advancing the future of home care and hospice at Catholic Health.

Meet Giuliana, a 14-year-old Girl Scout who's bringing comfort and color to others through her Girl Scout Silver Award Project!

Inspired by visiting her uncle at the Good Shepherd Hospice Inpatient Center in Port Jefferson, Giuliana created a heartwarming coloring book for children visiting patients—bringing smiles, comfort, and a little creativity to their day.

Her compassion and generosity are a true inspiration. Thank you, Giuliana, for showing us that kindness knows no age!

It's Founders Day!

120 years ago, an apple was given as a gesture of friendship and welcome to three Sisters of the Infant Jesus who had been forced to leave their native France because of anti-religious legislation.

On October 21, 1905, these migrants to America were first befriended on the piers of New York by a small boy who suddenly presented each Sister with a gift of fruit from their new world.

Every year, on the anniversary of that day, apples are given to Christ’s sick of every age in their homes and in the hospital. A symbol of God's providence and the fruitful work of the Congregation of the Infant Jesus Nursing Sisters of the Sick, Poor.

October is Physical Therapy Month!

This month, we proudly honor the incredible work of our Physical Therapists and Physical Therapy Assistants who serve with compassion, expertise, and unwavering dedication.

We are truly blessed to work alongside such a committed team — professionals who work tirelessly to treat, restore, and improve the quality of life and well-being of our neighbors across Nassau, Queens, and Suffolk counties.

Please join us in recognizing and thanking our PTs and PTAs — not just this month, but every single day — for the amazing difference they make in the lives they touch. 💙
