Basin Coordinated Healthcare offers several health services including:

Personal Care Services- This program is for eligible Medicaid recipients over 21 who have a medical need for services. Through this program, a client may choose a caregiver to assist them with the activities of daily living and help keep them independent in their home. Medically Fragile Waiver Program – This program is for eligible Medicaid children and adults who are at risk for/or have a developmental delay or disability and in a medically fragile condition. Home health aides and skilled nursing are offered through this program. Early Periodic Screening, Diagnosis & Treatment (EPSDT) Program – This program is for eligible Medicaid recipients under 21.
