MINOR SURGERY 10-3-25 Dr. Hal London

Today I present for you a small procedure to remove a facial cyst. This cyst is commonly called a sebaceous cyst, but in reality it does not contain sebum( skin oil) rather it contains keratin and is more accurately called an epidermal inclusion cyst.

Epidermal inclusion cysts form from blockage to a hair follicle (an opening in your skin where hair grows out) at the top part of the the follicle.
Your body naturally sheds skin cells. If you have a skin injury like a scratch, surgical wound or a skin condition like acne or chronic sun damage, it can alter the path your skin cells shedding. This traps these cells and other components like keratin, so they collect under the surface of your skin creating a cyst.

The procedure is rather simple and actually kind of fun. After sterile skin prep and injecting local anesthetic, a single small incision over the cyst is made deep to the top of the cyst , but not cutting into it. The cyst is then carefully dissected out in one piece to include the capsule around it.

It appears as a small spherical mass about the size of a marble in most cases( see photo). On occasion, it is not possible to remove it intact. When the cyst ruptures the very foul smelling contents must be expressed and the capsule dissected then removed. The consistency of the contents is that of ricotta cheese, but the horrible smell is a cross between Limburger cheese and bad foot odor. Once smelled, never forgotten.

The wound can then be closed with one or two stitches, which will be removed in 7 to 10 days. The scar will be nearly invisible if the surgery is performed in this manner.

UPPER LIP BARCODE LINES - Dr. Hal London

They are caused by repeated motion. Repetitive skin folding, be it from smoking, use of straws while drinking, or merely expression, can result in permanent creases. The areas of the face with the most motion will have the most lines ie. the perioral ( around the mouth) area in most patients. More motion = more lines. They are also called smokers lines for obvious reasons.

These vertical upper lip lines (resembling a barcode ) affect women more often than men. Men have course facial hair which adds support to the skin and is thought to be protective.

Treatment

Treatment can involve several modalities. During the consult, I teach behavior modification such as stopping smoking, or changing how you puff on a cigarette or draw from a drinking straw. A straw attachment shown in photo might be helpful.

Most patients have no idea that expressions causes facial lines. During the consult, I’m watching how my patients move their face. I watch them speak and express themselves then explain to them how their expressions are causing facial lines.

Behavior modification along with physical modalities like laser, filler, and toxin are used singly or in combination to achieve my results.

In the photos presented today the first photo shows her mouth in a resting position. Notice the vertical upper lip lines that visibly flash when she purses(second photo). The bottom photo was taken after treatment. Notice the softening of the lines. Nice.

FACIAL REJUVENATION - Dr. Hal London

These photos of my patient were taken 7 months apart. She first came to see me in November of 2024. At that visit we discussed a game plan to help with some age related changes in her appearance.

What was done?

Allow me to show you. Start at the hairline and work your way down, visually scanning the before and after photos.

In her forehead, can you appreciate how her resting face looks more relaxed, calm, and at ease? This is Dysport and filler at work.

Between her brows and lips do you see how the midface looks subtly lifted? That’s midface filler.

In the area just below the nose to the chin, can you see how turning up the mouth corners, filling the sad marionette lines, and adding subtle volume to the lips take years from her lower facial appearance ?

Lastly, she had an aggressive CO2 laser to help with fine lines and age spots.

Then end result is a natural looking enhancement that should last a year or more.

MINOR SURGERY 9-22-25- Dr. Hal London

The procedure presented today is removal of a scalp cyst( pilar cyst). These are rewarding little procedures that are actually fun for me and yield great cosmetic results for my patients.

A pilar cyst, also known as a trichilemmal cyst, is a harmless, non-cancerous lump that develops on the scalp from the outer sheath of a hair follicle and is filled with keratin, the same protein that makes up hair and nails. These smooth, firm, mobile, and round cysts can be solitary or multiple, may run in families.

Inside of these cysts is a cheesy, foul smelling substance( think foot odor mixed with Parmesan cheese). They may become irritated and infected but usually are just unsightly with patients wanting them removed for cosmetic reasons.

Scalp surgeries usually present no problems because the blood supply is rich making them less prone to infection and in hair bearing areas the scar will be hidden. Even if there is no hair to hide the scar proper technique creates the smallest scar.

After numbing the area a single, small, superficial skin incision is made over the cyst deep to the top of the cyst but not into the cyst. This way the cyst can be dissected out intact to include the capsule that surrounds it(see photo). If the cyst ruptures during the procedure the foul smelling contents( consistency of cottage cheese) can be expressed then the capsule dissected and removed. If the capsule is not removed the cyst will reoccur.

Skin cysts, similar to the pilar cysts, can be found on the skin anywhere on the body and are removed in a similar fashion.

It’s that time of year again. When cooler weather sets in, it’s the time to get your skin resurfacing.

In fact, I just had my skin laser treated two days ago. No down time for me. I’m still seeing patients every day and I use the visible changes during my healing process to teach patients about skin resurfacing.

Most of my patients are Florida residents. Many have had significant sun exposure since childhood and their skin reflects it. Dark spots, premature wrinkling, and altered skin texture are just a few of the effects of sun exposure over years.

My carbon dioxide laser has many options for treatment settings. You can opt for a mild treatment with a couple of days of redness or an aggressive treatment with four or five days of recovery, and anything in between. A mild treatment gives you a nice glow, helps with pigment, and fine lines. An aggressive treatment removes years of sun damage in one session, can lift dark spots, and remove fine lines and wrinkles.

FILLER AND TOXIN- Dr. Hal London

Forehead lines are mostly from expression. You see them as horizontal lines across the forehead and vertical lines between the brows. Raising your brows and frowning are the motions that cause these lines. When you can see the lines without making the expression, they are etched in skin similar to scars and are permanent without treatment.

Why treat them? Facial lines project emotion to others. Frown lines give you the angry/stressed look. Forehead lines make you look worried and aged. In the lower face marionette lines give you a sad/angry look.

Good news is that skin heals if you allow it. Skin cells are constantly sloughing invisbly and new surface cells replace them.

Toxins like Botox and Dysport weaken muscles of expression temporarily allowing the skin to heal. If you keep the muscle paralyzed by getting toxin every several months the etched lines can soften( slow process).

Most patients don’t want to wait a year for toxin alone to improve the etched lines and opt for filler in them as well. Filler will yield instant results and in this area the results are lasting.

Properly selected filler must be injected precisely to assure safety and great aesthetic results.
