Your resting face says something to the people you encounter. It projects who you are when you are not smiling. With age, the mouth corners turn down and can give you a sad or mean look.
Even when you are not feeling it, you may be looking it.

One of my goals is to always remove the negative from a face. Like treating the scowl between the brow with Botox, I support the corners of the mouth with filler to remove the frown and change the way the face projects to the world.

In the patient presented today, I also gave her lips a bit of attention to restore her youthful contours and volume.

The combination of lip filler with filler support under the mouth corners is a real winner in my practice. When performed with precision and finesse, it removes years from the face in a very subtle way.

FILLER FIX- Dr. Hal London

She came to me as a new patient wanting filler removed. She had lip filler injected about a year before by someone else.

Anatomy lesson: The lips transition from the dry outer portion to the wet mucosal portion as you head towards the inside of the mouth. The junction of the wet and dry areas is called the wet-dry border. Filler is best placed on the dry side of this wet-dry border.

If you look at the first photo, you can see the bubbles of filler on the wet portion of the upper lip. She wanted her lip filler dissolved then have the lips refilled properly by me later.

Check out the after treatment photo. The filler is gone 30 minutes after I dissolved it with enzyme . I added filler to her lips two weeks later and she loves it.

Be careful who you allow to inject you. Remember, it’s the Filler🧑🏻‍🦱not the filler that makes the biggest difference in your results.

Results like this are typical of what I do. I target the areas of facial aging and rejuvenate them. My goal is to take 5-10 years off the face while maintaining a natural appearance.

Toxin lasts 3 months in most cases. The filler can last much longer, often more than a year.

I like to see my patients back at two weeks for photos and touch ups if needed. I don’t charge for the product like most practices commonly do. My filler and toxin is free! I do charge per area treated so if you need a touch up at two weeks that will most often be free as well.

My practice is unique. All treatments are performed by a board certified physician( me! ). I handle my own schedule, no other staff to deal with. You have a direct connection to the person doing your treatments. This makes it very easy to get an appointment or to change an appointment. If you have concern during the weeks after your treatment you will have my number to text or call. Leave a message and I’ll make every effort to call you back before the day ends.

We live in a time when personal service is but a memory of days past. I get frustrated when I have to deal with phone tree type of electronic messages, having to speak with multiple layers of staff before I get my questions answered, and ambiguous billing practices.

We can and should do better and my practice is designed to take you back to a simpler time when the doctor- patient relationship had real meaning.

FILLER FEAR #1- LOOKING WIERD!

I hear it all the time, “ I dont want to look wierd”. Patients know that filler in the wrong hands can ruin your looks. We’ve all seen that person in the grocery line or the restaurant who looks part human/ part alien from another planet.

Do you wonder, like I do, if this person has a distorted sense of self? That over-done look does nothing but project insecurity and low self esteem.

Even worse is the doctor who will enable this patient in their pursuit of the impossible( eg. looking 30 when you are actually 70). If a patient wants a treatment and has the money, game on.

I say NO, a lot. Not trying to be mean but trying to save them from themselves. Filler injected improperly or in excess can ruin their looks. I won’t do that.

Natural looking, age appropriate treatments are my forte. I’ve built a career on it.

HAND REJUVENATION - Dr. Hal London

Want to tell a person’s age? Look at their hands!

Unless you wear gloves all the time, hands are exposed to the elements. Sun exposure will cause dark spots, rough scaly patches( precancers), and thin, easily torn skin. I use my laser and peels to remove these surface defects.

MINOR SURGERY 10-3-25 Dr. Hal London

Today I present for you a small procedure to remove a facial cyst. This cyst is commonly called a sebaceous cyst, but in reality it does not contain sebum( skin oil) rather it contains keratin and is more accurately called an epidermal inclusion cyst.

Epidermal inclusion cysts form from blockage to a hair follicle (an opening in your skin where hair grows out) at the top part of the the follicle.
Your body naturally sheds skin cells. If you have a skin injury like a scratch, surgical wound or a skin condition like acne or chronic sun damage, it can alter the path your skin cells shedding. This traps these cells and other components like keratin, so they collect under the surface of your skin creating a cyst.

The procedure is rather simple and actually kind of fun. After sterile skin prep and injecting local anesthetic, a single small incision over the cyst is made deep to the top of the cyst , but not cutting into it. The cyst is then carefully dissected out in one piece to include the capsule around it.

It appears as a small spherical mass about the size of a marble in most cases( see photo). On occasion, it is not possible to remove it intact. When the cyst ruptures the very foul smelling contents must be expressed and the capsule dissected then removed. The consistency of the contents is that of ricotta cheese, but the horrible smell is a cross between Limburger cheese and bad foot odor. Once smelled, never forgotten.

The wound can then be closed with one or two stitches, which will be removed in 7 to 10 days. The scar will be nearly invisible if the surgery is performed in this manner.

UPPER LIP BARCODE LINES - Dr. Hal London

They are caused by repeated motion. Repetitive skin folding, be it from smoking, use of straws while drinking, or merely expression, can result in permanent creases. The areas of the face with the most motion will have the most lines ie. the perioral ( around the mouth) area in most patients. More motion = more lines. They are also called smokers lines for obvious reasons.

These vertical upper lip lines (resembling a barcode ) affect women more often than men. Men have course facial hair which adds support to the skin and is thought to be protective.

Treatment

Treatment can involve several modalities. During the consult, I teach behavior modification such as stopping smoking, or changing how you puff on a cigarette or draw from a drinking straw. A straw attachment shown in photo might be helpful.

Most patients have no idea that expressions causes facial lines. During the consult, I’m watching how my patients move their face. I watch them speak and express themselves then explain to them how their expressions are causing facial lines.

Behavior modification along with physical modalities like laser, filler, and toxin are used singly or in combination to achieve my results.

In the photos presented today the first photo shows her mouth in a resting position. Notice the vertical upper lip lines that visibly flash when she purses(second photo). The bottom photo was taken after treatment. Notice the softening of the lines. Nice.
