Dear patients and families

Starting November 1st we will be transitioning to a new electronic medical record system that will help us serve you better. We kindly ask for your patience during this time as we learn the new system to better meet your needs and ensure safety of your medical care and information.

Please note:
-Our office will be closed on Nov 1 and Nov 3 for training. Phone calls will be limited to triage on call nurse only. In case of emergency, please call 911, visit the nearest ER or urgent care office.

-The patient portal will be fully deactivated on Oct 27th and we will gradually transition to MyChart. If you need a refill – please message on portal before Oct 27th , later – please call the office directly.

We appreciate your patience and understanding. We are striving to make this transition as smooth as possible.
