When people talk about a “biological clock,” it’s usually about women.

But here’s something many people don’t realize:

Men have a biological clock, too.

While men continue producing s***m throughout their lives, s***m quality and fertility decline with age. In fact, when a heterosexual couple experiences fertility challenges, male factors are involved about half of the time.

Yet historically, women are often tested first.

That conversation is changing.

What Happens to Male Fertility with Age?

• S***m count gradually declines
• S***m motility (“swimming ability”) decreases
• Semen volume drops
• More s***m show DNA damage
• Time to conception increases

Research shows men over 45 may take significantly longer to conceive compared to men under 30. There is also a higher risk of miscarriage linked to increased s***m DNA fragmentation in men over 40.

It’s Not Just About Age

Lifestyle and environmental factors also impact s***m health, including:

• Smoking
• Excess alcohol use
• Obesity
• Sedentary lifestyle
• High processed food and sugar intake
• Exposure to environmental toxins

Medical conditions such as varicoceles (enlarged veins in the sc***um), hormonal imbalances, or structural issues can also affect fertility, many of which are treatable.

Fertility Is a Shared Journey

The American Society for Reproductive Medicine (ASRM) and the World Health Organization both recognize that infertility should be evaluated in both partners.

If you’ve been trying to conceive for 6–12 months (or sooner if over 35), both partners should be evaluated at the same time.

This question comes up often when couples are trying to conceive.

Here’s what current research actually tells us:

A recent population-based study looking at couples attempting pregnancy found that there is still a significant data gap regarding:

• Underwear type and semen quality
• Whether switching to looser underwear impacts s***m parameters
• Effects on time-to-pregnancy
• Risk of infertility

Importantly, no direct measurement of scrotal temperature was performed in the study.

At this time, there is no definitive evidence that simply changing underwear type improves fertility outcomes.

Male fertility is influenced by many factors, including hormones, overall health, medical conditions, and lifestyle.
