Hello!

I've accepted a primary care position at NACA (Native Americans for Community Action) family health center that now allows me to take insurance! For the time being I am only rarely accepting patients at Proximity Primary Care and focusing my available time on my existing clients. If you would like to be considered for my direct primary care practice you can email me your name, date of birth, and primary medical concerns and I will review these on a case by case visit. A lot of why I started my practice was to have longer visits and good communication with my patients. NACA provides this for me while also allowing me to take insurance and see medicaid/medicare patients. Why are medical visits expected to be no more than 15 minutes?
Why do providers address one or two issues at a time instead of evaluating you holistically?
Why is it so difficult to contact your healthcare provider directly? \
Why don't providers do home visits?

Unfortunately, the answer to these questions is that it takes 2000+ patients to "fill" a doctor or nurse practitioner's schedule. In order to keep most practices afloat providers need to think about what the insurance companies will reimburse and not just about what their patients need. I've worked in the US medical system for 5 years and I can no longer pretend that 15 minutes is an appropriate amount of time to spend with a patient.

The solution? Direct primary care! Direct primary care is a movement in the primary care industry towards boutique medicine. Instead of taking insurance I charge affordable rates of $75/month for unlimited visits, texts and calls. Each of my visits are allotted at least 60 minutes of time. My goal is to take care of 100 patients as well as I possibly can instead of struggling to rush through a 2000 patient panel. I am a family nurse practitioner with over 5 years of experience in primary care, chronic disease management, addiction management, migraines, women's health, weight management, gender affirming care, HRT, diabetes, and a wide variety of acute and chronic ailments. I offer same day, evening, and weekend appointments upon request. I offer office visits, telemedicine visits, and home visits. At proximity primary care, I offer direct primary care. This means that each appointment is given at least 60 minutes of time. There are no long wait times and each of my patients has my direct contact information so that I am always available for them. I don't take insurance because insurance companies insist on 15 minute visits and addressing only a few issues at a time. Insurance companies require staff which inherently separates me from my patients. Insurance companies require a panel size of 2000 patients. Instead I work alone and I charge $75 a month for unlimited texts, calls, office visits, and home visits. Office visits are available in a convenient downtown location. I offer evening, weekend, and same day availability. Instead of taking care of 2000 patients in 15 minute increments I want to take care of 100 patients in the most careful and thorough way possible.

I have over 5 years of experience in chronic disease management, diabetes, COPD, weight management, anxiety, depression, insomnia, addiction, opioid use disorder, gender affirming care, women's health, men's health, heart disease, and a wide spectrum of other acute and chronic illnesses.
