We Humans are homosapiens , an upright walking species. Bipedalism is what really defines us apart from intelligence. To be able to hand over this species defining trait back to bed ridden neurological patients through surgical skills and clinical judgment is what makes me as a neurosurgeon to be proud off. Sharing 3 videos of patients of varing ages each suffering from debilitating neurologic disease walking back on their feet after different neurosurgical procedures. There is nothing more defining than "To be able to stand upright"

Regaining every breath of life from jaws of near certain death. Overcoming two deadly adversaries simultaneously - Covid 19 pneumonia and brain hemorrhage.
This 66 years old gentleman was diagnosed to have Covid 19 pneumonia. 2 days later he developed severe headache, progressive drowsiness and left hemiparesis. His CT Brain revealed massive brain hemorrhage. He was shifted from Gulbarga in a critical state. He was immediately intubated in ICU and then underwent emergency evacuation of the bleed. The surgery went off well and he was extubated on day 2. On day 5 as his oxygen levels dropped and his CT showed worsening of pneumonia, he hung onto life with amazing relisence. On Day 8 his oxygen level began to improve and it was then that we realised that he would survive this deadly assault on his lungs. Over the next 1 week he made a remarkable recovery neurologically and was finally discharged.
In order to overcome both these adversaries, the entire universe needs to conspire. I believe most often the healing hands of doctors, the magic of modern medicines, advances in surgery and faith in God makes a significant difference in shaping such patients destiny.
A covid and brain hemorrhage survivor at the centre with covid warriors around him. We will relish these moments of satisfaction a long time inspite of the chaos surrounding us. More than a year into the pandemic and it has taught us few life lessons - staying humble, being kind and grounded.

Yet another young life transformed !!
Spine surgery at its very best.. Transforming the life of this young girl from a seemingly painful bed ridden state to a pain free ambulant independent state.
This 18 years old girl presented with 6 months backache which had gradually worsened. Since last 1 month the pain had rendered her bed ridden and she experienced weakness in both her legs (power grade 2/5). Her MRI spine showed D10 and L4 kochs spondylodiscitis with epidural abscess and paravertebral abscess. Surgery was planned in two stages. In the first stage 4 level pedicle screw fixation, D10 corpectomy and D9 to D11 interbody mesh fusion was done. 4 days later second stage L3, L5 bicortical pedicle screws, L4 corpectomy, L3-L5 mesh interbody fusion was done. Each surgery took nearly 5 hrs. The culmination of these two beautiful surgeries was evident 6 days later when she started walking again albeit with some support. The smile on her parents face when she took her first baby steps after nearly 2 months made our day.

We possess within ourselves at every moment of our lives, under all circumstances, the ability to transform lives for good. I would say KEEP THE FAITH. In gratitude as always.

Here is the follow up of this 18 years old girl having undergone spinal surgery for excision of 16 cm spinal tumor 3 months back. From seemingly hopeless, dependent state to regaining, independent, meaningful life with a purpose in nearly 2 months, is enough reason to continue rendering our medical services in these extraordinary and difficult circumstances. Sharing another interesting case of multicentric/multifocal frontotemporal GBM in a young 28 years old male presenting with seizure. He had undergone frontotemporal decompressive craniectomy and biopsy of the lesion at a remote hospital. Further resection was not attempted there due to dismal nature of the disease. In view of recurrent seizures and persistent headache, gross total resection was done followed by radiotherapy.
The challenge during surgery was to achieve gross total resection in a multicentric glioma without causing neurological deficit.

*Good deeds are never lost*

"Every time you do a good deed in the past or present, you shine the light a little farther into the unknown darkness of future"

These trying circumstances today has given me ample of time to look over my shoulders into the past. A few days back, I was very surprised and at the same time overwhelmed to see one of my very old patient walk in my consulting room late afternoon. He was my first neurosurgical patient in this town and hence a very special one. One that enabled me to spring leap into Solapurs neurosurgical fraternity, One who entrusted me with his life on that terrible night, one who gave me that small window of opportunity to prove that I was good enough, one who became my responsibility to deliver the best surgical result that night. Today as I look back, I am delighted to realise that I had delivered then.
This 40 years gentleman, a merchant in pune had come on the night of 12th Feb 2012, a week after starting my neurosurgical career in this textile town- Solapur, following a fall with intracranial bleed. He needed an emergency brain surgery for his survival. The brain clot was evacuated at 3 am in the night after persuading our anesthetist to come over. He made a remarkable recovery after surgery. After his discharge from the hospital, he never came back until now 8 yrs later. As he walked over in my consulting room with a smile on his face, introducing himself, those memories of initial struggle in my career instantly flashed back. Over the next 15 min as we discussed how enough water has flown below the bridge, it was a pleasant remembrance of how far I and he have come.
A lesson learnt --
Never forget the ones who helped you reach where you are today, no matter how small or significant their contribution was. Life always finds a way to get back to you.

Decision making is a crucial factor in neurosurgery. Time to hospital admission is equally important in moderate to severe head injury patients. Its always a race against time in these patients. As the clock keeps on ticking, with every second lost, the fine line between life and death gets blurred.
This 40 yrs old lady presented yesterday with left frontal hematoma following fall. Left pupil was dialated, right was reacting to light. She underwent urgent evacuation of left frontal hematoma. Time of admission: 5.30 PM. Time of starting surgery: 6.30 PM. Surgery was done with by 8 PM. Quick decision making, swift response from the entire team on Sunday evening has made a difference in patient outcome !! Exhilarating experience to see the patient come alive from near certain death if had not intervened.
