The cash price for Lp(a) through Fullscript/Quest is $16. So let’s stop pretending cost is the barrier.

Lp(a) is not theoretical. It is an inherited LDL-like particle with an apolipoprotein(a) tail that impairs fibrinolysis, carries oxidized phospholipids, promotes endothelial dysfunction, and independently amplifies cardiovascular risk. That is not esoteric molecular trivia. That is vascular biology.

The question should not be, “Is there a mortality RCT where lowering Lp(a) alone saved lives?” The question should be, “If this particle clearly amplifies risk, why would we choose not to measure it?”

We routinely order tests that cost far more than $16 without hesitation. We prescribe lifelong medications without knowing whether an inherited risk amplifier is present. Yet somehow this one test is labeled “too expensive” or “unproven?” 😒

Sixteen dollars is not the issue.

What is uncomfortable is stepping outside a narrow evidence hierarchy that waits for pharmaceutical-sponsored outcome trials to tell us what to do before acknowledging biology. Many of those trials underrepresent women. They underrepresent Black patients. They underrepresent the diverse populations we actually treat. And they often arrive only after a marketable drug exists (for obvious reasons). 🤑

How many women are on a beta-blocker right now, faithfully taking a medication prescribed “by the guidelines,” believing it is protecting their heart, when newer data suggest that in contemporary post-heart attack patients with preserved or mildly reduced ejection fraction there was no reduction in cardiovascular events overall and women in some analyses experienced more events, particularly when treated with beta-blockers? For decades this class has been reflexively prescribed after MI and widely used for blood pressure control, based on trials built largely in male cohorts and then generalized to women without meaningful sex-specific validation. [REBOOT trial, European Heart Journal, Volume 47, Issue 6, 7 February 2026]

In the same way...

Ignoring a clearly established risk amplifier because it lacks a single hard endpoint RCT is not caution. It is a choice to operate with incomplete data when more complete data is inexpensive and available.

Mortality is not the only meaningful endpoint, it is a LATE endpoint. Risk amplification happens first and should be measured. Death is binary. Cardiovascular risk is not.

And pretending a $16 test is financially prohibitive in modern medicine is simply not an honest argument. It's not. The decision we are making is whether we are willing to look upstream...or not.
