Lakeland Care coordinates care for eligible elders and adults with disabilities in 31 counties. Lakeland Care, Inc.

(LCI) is a Wisconsin-based managed care organization (MCO) providing long-term care services and supports to eligible frail elders and individuals with physical and intellectual/developmental disabilities through Wisconsin’s Family Care program. Lakeland Care’s goal is to support individuals to live as independently as possible in their homes and communities by providing high quality, cost effective services and supports using a member-centered, outcome-based approach. Self-enroll with Medicaid now.

Medicaid payments will end in 2026 for providers of Medicaid waiver authorized services who do not obtain a Medicaid ID number by self-enrolling with Wisconsin Medicaid through the ForwardHealth Portal now.
