-Provide triage telephone support, assessing patient needs and answering questions.
-Offer clear instructions and education based on established guidelines.
-Ensure high-risk patients receive timely follow-up care after hospital discharge.
-Educate patients on contacting their physician or calling 911 for urgent matters.
-Process discharged patients from the Emergency Department, ensuring they receive appropriate follow-up.
-Assist with outreach calls and campaigns to enhance patient engagement and support the Quality Department’s goals.

Please see below for the PHCN Network Thanksgiving hours:

PHCN North, North Pediatrics, Admin, and Gastro offices will be CLOSED on Thursday, November 27 and Friday, November 28.

PHCN Macomb will be CLOSED Thursday, November 27 and Friday, November 28 but will be open the following weekend (11/29 and 11/30).

PHCN South and Marysville offices will be CLOSED Thursday, November 27 but will remain OPEN on Friday, November 28 with regular business hours.

All offices will resume regular hours on Monday, December 1.
