Comprehensive Cancer center that offers multiple options.

Avoid cancer centers that only offer external beam radiotherapy (EBRT). Although EBRT is an excellent form of therapy, you deserve a center that offers prostate implants. Only the best radiation oncology centers offer BOTH forms of therapy.

Prostate implants are also known as brachytherapy. There are two main forms of brachytherapy: low-dose rate brachytherapy (prostate seed implants) and high dose rate brachytherapy. (I will write another blog post dedicated to brachytherapy).

Having a highly trained physician in charge of your care is very import, but other members of the team as also critical in maximizing a successful treatment.
Your center should have a PhD level physicist heading the physics department. A top level physics team will ensure that the machines used to deliver radiation (linear accelerators) are operating at maximal efficiency and the plans produced by the physician can be delivered as intended.

Accounting For Organ Motion.

The prostate actually moves based on re**al and bladder filling. This movement can affect the accuracy and precision of radiation delivery and cause “geographic misses”. These “misses" result in under-dosing of the prostate and over-dosing of the bladder and re**um. This can compromise cure rates and increase side-effects of therapy. In order to avoid this, we use Cone beam CT (CBCT). CBCT is a CT scan built into your radiation machine. CBCT acquires a 3D CT image of your prostate in real-time just before treatment.

Intensity modulated radiotherapy (IMRT)

Intensity-modulated radiation therapy (IMRT) is an advanced form of external beam radiotherapy. IMRT allows for extremely accurate delivery of radiotherapy, which results in higher doses of radiation to the prostate and lower doses to other tissues such as the bladder and re**um. High quality CT simulation for IMRT treatment planning.

A dedicated CT Scan of the pelvis is required to plan EBRT. Your radiation oncologist uses this CT Scan to accurately delineate the prostate and avoidance structures (re**um, bladder, pe**le bulb and femoral heads).

Without a state of the art CT scan, delineation of these structures will be inaccurate. Avoid facilities with CT scans over 5 years old, those who do not have a CT scan that is not dedicated specifically for radiation oncology or radiation oncology facilities that do not have their own CT scan located within a radiation oncology department.
