SW FL interventional pain management physician Jonathan S. Daitch, MD discusses the positive results of the MILD (minimally invasive lumbar decompression) and MinuteMan procedures with a recovering patient at her post-op appointment.

Evelyn had previously experienced difficulty walking or standing, enduring miserable pain that ran from her lower back down her leg. In this interview, Dr. Daitch reviews how the removal of a small amount of offending ligament and implantation of a stabilizing device - the combination of two minimally invasive procedures performed through a 1-inch incision - have given the patient restored comfort and ease of motion.

Now excited about resuming a life where day to day tasks are performed with ease, Evelyn is especially pleased to know she can cook again and stand for long periods without experiencing pain.

MILD (minimally-invasive lumbar decompression) is an outpatient procedure that relieves pressure in the spinal canal through an incision the size of a baby aspirin. This outpatient procedure is quick (about 30 mins) and covered by Medicare nationwide.

Results are typically experienced within a few days, providing:
• Reduction in pain
• Restored mobility
• Increased standing time of up to an hour
• Increased walking distance

Minuteman is performed through a one-inch incision and does not involve cutting any muscles or bone. There is minimal blood loss and you return home the same day - generally within an hour after the procedure ends. There is also minimal discomfort after this type of stabilization procedure.

In this, a heavy-duty toggle bolt (with wings) is placed through the rear elements of the spine. Once the bolt is placed, the toggle wings are opened. The bolt opens the spinal canal and creates room where the pinched spinal nerve roots exit. The wings catch on the spinous processes and a “nut” tightens everything into position, stabilizing the spine. Bone cement is added to further ensure proper fusion/stabilization of the spine.

The device is a permanent implant and can be surgically removed in the future if needed but does not burn any bridges should further treatment be necessary.

Minuteman can be especially beneficial for patients who have been offered more extensive surgical fusion procedures and prefer a less invasive option or for patients who may be unable to withstand more complex in-hospital procedures as well. It is also an excellent option for patients with single level disease (for example L4-5 or L3-4) who meet certain criteria of spinal stenosis, or spinal slippage (spondylolisthesis).

Although symptoms may not be obvious just after a MOTOR VEHICLE ACCIDENT (MVA), the extreme force can severely impact the neck and lower back. Early treatment can minimize the long term effects on spinal anatomy.

Common injuries after an MVAs are:
- Cervical (Neck) Whiplash - When you are impacted from the rear, your neck is forcibly thrown backwards into the headrest. It then snaps forward with the chin hitting your chest, followed by a significant rebound backwards. This can result in severe headaches (cervicogenic headaches) that radiate from the sides of your neck to the back of the neck OR upper neck pain that causes headaches at the base of the skull.

- Lumbar (Lower Back) Disc Injury - In this, MRIs often show bulging, a tear in the disc, or a disc protrusion/herniation. Treatment may begin with lumbar epidural injections, transforamenal injections, and associated physical therapy. If these measures fail to provide sufficient relief, additional imaging may be ordered to determine the precise location and extent of the injury.
For further treatment, a procedure known as DiscFX® may be recommended. We also offer VIA Disc, which helps to repair injuries to the disc’s interior. Finally, surgery may be indicated if severe lower back pain and associated sciatica does not sufficiently respond to minimally invasive treatment.

The key is to be seen by a board certified pain management physician soon after an MVA. Through early treatment and proactive measures, you may be able to avoid years of resulting pain. Some studies show that wherever your pain is at the 6-month mark is where it remains long term (without treatment).

SPINAL STENOSIS occurs when the spinal column's inner core narrows, which causes pressure on nerves. This can lead to pain, numbness and weakness in the legs, and other symptoms.

Lumbar Spinal Stenosis (LSS) causes low back pain that is typically accompanied by:

• Pain, weakness or numbness in the legs, calves or buttocks
• Cramping in the calves with walking, requiring frequent short rests
• Pain radiating into one or both thighs and legs, similar to sciatica
• Pain relief when bending forward, sitting or lying down

to Jonathan S. Daitch, MD on his presentation "How MINUTEMAN Treats Degenerative Instability" on October 22nd.

The presentation, held at Connors, was sponsored by SpinalSimplicity. In his presentation, Dr. Daitch discussed the option of the MINUTEMAN procedure to the Interventional Pain Community with neurosurgical colleagues present as well.

MINUTEMAN is a minimally invasive procedure that treats symptoms related to spinal stenosis, such as:

Disc degeneration can lead to spinal stenosis, causing a decreased ability to walk more than a block or two or stand more than 10 - 15 minutes. It is often accompanied by buttock and leg pain that worsens with standing and walking.

MINUTEMAN, a minimally-invasive spine stabilizing device (fusion) procedure, is performed through a one-inch incision in about 25 - 30 minutes. (This is compared to the usual 3 - 4 hour spinal fusion surgery time when performed at a hospital followed by a 2 - 3 month recovery.)

The MINUTEMAN procedure can be especially beneficial for patients who have been offered more extensive surgical fusion procedures and prefer a less invasive option or for patients who may be unable to withstand more complex in-hospital procedures.

And, because Minuteman does not involve cutting any muscles or bone, recovery is quick with minimal post-op discomfort.
