If you have been trying to build something bigger than your current job description and the people closest to you do not understand why — that loneliness is real and it is one of the hardest parts.

Building a practice or expanding a service while managing a household & a full caseload is extremely difficult and draining!

But the hardest part is feeling like your people cannot quite see the vision that you have — so the marathon that you feel like you’re running often doesn’t include high fives or cheers of support.

Up-leveling your practice in the midst of juggling all the things requires a community of like minded professionals who understand the isolation, fatigue, & are learning how to grow while maintaining boundaries and operating within their core values too.

Carrying the vision, walking through the challenges and set backs within in the right community changes everything.

Your patient has a right to thorough swallow studies! Here’s what that looks like in practice… Advocating for another swallow study after a patient has already had a repeat VFSS while in the hospital can be challenging!

Educating your team is an ongoing process and requires a strong commitment to improve therapeutic outcomes. It also requires having a good working relationship as a foundation for building strong systems to support dysphagia practice.

Thorough bedside evaluations and data collection are essential to be able to present your case in a positive and professional way.

This outcome is one of many FEES that have led to an unexpected result. I know I sound like a broken record but it’s further proof that it’s impossible to know what’s happening inside the patients throat!

It’s not always the outcome that the patients want but it’s full of the data necessary to make the best decisions for the patient!

Save this for the next time you need to make the case for another swallow study and you need the words to do it!

We walked into yesterday’s meeting with Arkansas BCBS prepared for resistance.

What we found instead was a room full of senior leaders who asked thoughtful questions, told us they’ve already been working toward solutions, and committed to staying at the table with us.

Arkansas Speech Language Hearing Associations (ArkSHA) committee devoted to payer advocacy has been working on issues with Arkansas BCBS.

Here’s what came out of the meeting this week:
⭐️30-day follow-up committed
⭐️Quarterly ongoing meetings established — virtual or in person
⭐️Our concerns were heard at a senior leadership level.

This is not the end.

It’s the beginning of an ongoing partnership — and that is exactly what sustainable advocacy looks like.

To every Arkansas SLP who has voiced frustration about recoupments, reimbursement rates, and feeling unheard by BCBS — your concerns were in that room yesterday.

Major update for Private practice SLPs in Arkansas. ArkSHA heard your voices and we had been working behind the scenes to advocate for changes from AR BCBS through our Payer Advocacy Committee. We hit a major milestone by getting an in person meeting with Representatives at AR BCBS!

If you feel like an imposter every time you think about adding a new clinical skill, that feeling is not a red flag.

It is proof that you take your patients seriously.

If you’re a Med SLP who is holding back from pursuing FEES because imposter syndrome has convinced you that you are not ready, it’s more likely that you are worrying about getting it right and being the best therapist you can be and THAT makes you someone who is actually MORE equipped to do it well!

It sounds cliché, but being ready isn’t a feeling — it’s a choice. I’ve found that it’s best not to wait for the confidence. I’ve built it up by showing up anyway.

I started a mobile FEES business during a pandemic with no roadmap for operating a business during a global crisis and a lot of stubbornness. Here is what I know now that I wish I had known then.
Throughout those very scary early days of showing up even though I was absolutely terrified and felt completely out of my lane… the transformation happened despite my own fears and amid the chaos of the pandemic as I built relationships with SLPs and helped so many patients who wouldn’t have had access to a quality swallow studies. The pieces finally came together — I finally felt confident and able to do anything I focused on. Not overnight but after many days of showing up and doing the work. 🤍If you’re an SLP who can see the vision of what your practice could look like with FEES but cannot yet see the path- just take the first step. You don’t have to feel confident now and you don’t have to see the whole process. FEES competency is a long journey and it’s SO worth it. 🤍We need SLPs who are passionate about providing research backed diagnostics in our region!

The faces I make internally when someone tells me FEES is not necessary because the patient passed their bedside eval.

SLPs are brilliant and typically have a lot of information and expertise…. but we don’t have X-ray vision.

So many times our well meaning colleagues assume that we do.

Advocating for FEES /VFSS can often feel like you’re pushing a bolder uphill. Some days you want to scream and other days you’re crying in your car. 🤪

In those moments when it feels like you’re explaining the obvious… take a deep breath and remind your team that studies show 60% of aspiration is silent (no cough) and we can’t assess that at bedside.

We need to see the structures within pharynx and fully assess airway protection in order to give patients the best shot at recovery.

Share this with the SLP who has had to hold it together in exactly this conversation.

Mobile FEES of NWA provides (FEES) Flexible Endoscopic Evaluation of Swallowing in the River Valley, NWA and Southwest Missouri. FEES is a gold- standard, evidenced based and cost effective swallowing assessment which requires no radiation exposure, no barium consumption and can be preformed in the patients natural environment.

A Clinical Bedside Swallow Exam cannot properly diagnose dysphagia (the inability to swallow). Speech Language Pathologists (SLP) are AMAZING but we DO NOT have X-Ray goggles! Simply put, you cannot diagnose what you cannot see!
