If you have ever looked at a Swallow study report and thought 'this tells me absolutely nothing useful,' you are not the problem.

The report is.

I have seen a LOT of reports that have minimal info, no compensatory strategies, or info about underlying impairments and the worst is recommendations Ive seen have been “nectar TL because the patient demo pe*******on with thin liquids.” No description of where the pe*******on was or about the pts response.

A high quality report should leave you feeling empowered and outline…

Here is what is actually happening in your patient’s throat during a swallow and why you cannot see it without a scope.

FEES is a birds eye view of the swallow~
just like we are all different on the outside... our anatomy is also unique on the inside!

Without the ability to see inside your patients’ throat, you are missing important information, like the path of the bolus, the possible residue after the swallow, abnormal tissue, aspiration that does not trigger a cough response, & whether or not the patient is able to fully expel material that has crossed the plane of the vocal folds when they do cough with aspiration.

Quality swallow studies are not just an option for some patients with dysphagia... patients with pulmonary issues, HNC, & trach/vent patients... being able to identify airway patency & assessing the health and mobility of the vocal folds is a requirement!

Save this for the next time you need to make the case for why your patient needs more than a bedside eval.

I’m so proud to be a part of our state association Arkansas Speech & Hearing Association (ArkSHA) and to partner with Arkansas Otolaryngology Center, UCA Speech-Language Hearing Clinic to bring Ready, Set, Scope to Conway, AR!

If you have ever had a patient discharged before you could get them a proper swallow study and you are still thinking about it — that is NOT overthinking.

That is your conscience!!

I remember very well the weight of the patients I could not get the right assessment for in time. It’s a frustrating process to realize that not only do you have to treat the patients, manage politics within the therapy team, but also advocate for your patient for things that you assumed would be standard practice!

It’s draining!

Unfortunately our healthcare systems do not always move fast enough to help us support our patients.

Please understand that the discomfort you’re feeling is a sign of how much you care — not a sign of failure.

Prior to starting my FEES competency journey, I worked in skilled nursing facilities for over 16 years. I met a patient who changed my entire career.

He was a retired Methodist minister—one of those people who radiate gentleness & grace even in the hardest moments. During a difficult time in my life, he was the person who reminded me I was still whole & worthy of happiness.

He had a swallowing disorder & strict NPO orders. No ice chips, just the mouth swabs.

It makes me cringe to think about it. 🤦‍♀️

At the time, I didn’t know what I know now.
I didn’t have FEES.
I didn’t have mentors.
I didn’t have the clinical clarity I give my patients today.

That moment became the turning point of my life.

I promised myself never again.

Never again would a patient’s outcome hinge on lack of access or lack of training, lack of ability to advocate for my patients.

Never again would I be the clinician who “just went through the motions and did what I was told.”

That promise is why I went began the journey to become a Mobile FEES provider.

Fast forward to today and I know that one of the reasons I work so hard and strive to continue to improve daily is my own way of righting the wrongs from the past.

Now that I know “more” I can do better AND I can help others to find the best treatments for their patients so they don’t have to look back with regret when they are reminded of their sweet patients.

I’ve been working with patients navigating swallowing issues for 25 years, and this is something I see differently now than I did even a few years ago.
🤍I used to focus heavily on how many times a patient was coughing and with what consistency.

🤍Now I care much more about energy conservation and airway protection, because that is what determines long-term quality of life.

🤍When we only focus on stopping a patient from coughing , we often miss the fact that they are easily able to expel aspirated material from larynx, which is why they are able to keep drinking favored liquids despite a cough.

🤍If you’ve been blindly thickening liquids beside because a patient coughed with thin and thinking “why is this not working,” this shift may change how you approach dysphagia treatment.

A lot of SLPs feel unsure when they begin advocating for their patients and need support.
One resource that can help is creating a referral triage based on changes in the indicators found in Dr. Langmore’s research.

Mobile FEES of NWA provides (FEES) Flexible Endoscopic Evaluation of Swallowing in the River Valley, NWA and Southwest Missouri. FEES is a gold- standard, evidenced based and cost effective swallowing assessment which requires no radiation exposure, no barium consumption and can be preformed in the patients natural environment.

A Clinical Bedside Swallow Exam cannot properly diagnose dysphagia (the inability to swallow). Speech Language Pathologists (SLP) are AMAZING but we DO NOT have X-Ray goggles! Simply put, you cannot diagnose what you cannot see!

Mobile FEES of NWA is committed to being here for you! We are dedicated to being on site within 48 hours with our fancy, high-tech camera and when we leave you have the report in hand. No more difficulty reaching the SLP who preformed the study because we provide education and collaborate with nursing, SLP’s and family/caregivers during and after the study to ensure the highest level of service!
