Seasonal Affective Disorder (SAD) and obstructive sleep apnea (OSA) are two distinct conditions that frequently coexist and can worsen each other through interconnected biological and behavioral mechanisms. SAD is a recurrent depressive disorder that typically emerges during late fall and all of winter due to reduced daylight exposure, disrupting circadian rhythms, melatonin production, and serotonin regulation. Sleep apnea is a sleep-related breathing disorder marked by repeated airway obstruction, intermittent hypoxia, and fragmented sleep. When present together, these conditions create a reinforcing cycle of mood disturbance and sleep dysfunction.

Sleep apnea can intensify SAD symptoms by causing chronic sleep disruption, excessive daytime fatigue, and impaired emotional regulation. Intermittent hypoxia and systemic inflammation associated with untreated apnea have also been linked to worsened depressive symptoms. Daytime sleepiness may further limit physical activity and daylight exposure, both of which are protective factors against SAD. Conversely, SAD can negatively impact sleep apnea by promoting irregular sleep patterns, hypersomnia, reduced treatment adherence, and weight gain, all of which can increase apnea severity.

These interactions are particularly important in patients with cardiovascular disease, as both depression and sleep apnea are established risk factors for adverse cardiac outcomes. Health Information Alliance (HIA) plays a critical role in this context through its cardiac registry abstraction. By systematically collecting and validating clinical data for cardiac registries, high-quality abstraction allows healthcare organizations to better understand how overlapping conditions influence cardiac outcomes, quality metrics, and long-term patient risk.
Through standardized data abstraction and registry support, HIA can provide tailor-made services and solutions in its National Cardiovascular Data Registry, especially with Afib-Ablation. This data-driven insight supports more integrated care strategies, improved quality reporting, and targeted interventions.

Please join us in congratulating Liz Testa as HIA's December All-Star!

Liz began with HIA as a High School Intern, working on scanning HIA's contract files into the cloud. She interned again in college, writing articles to post on our website and social media. After obtaining her degree from Penn State several years ago, she joined HIA full time in the role of Digital Marketing & Public Relations Coordinator. Recently, she was promoted to Client Relations Manager. In her new role, she writes contracts and SOWs, handles all the planning, and attends HIA's conferences both on the Coding and Performance Improvement sides of our business. She interacts well with prospective clients and networks well with other vendors. Liz currently coordinates our student intern program for HIM students in collaboration with Rutgers, Palm Beach State University, and Camden County. She handles all the recruitment of new staff for Chris Grosso, conducts the initial interviews, and is using the related Paycom Modules. She works with Karen Oller when staff gets onboarded. She truly is an asset to our organization and further assists both Josh Stingel and Jeff Yanto in the course of their work loads. Congrats to Liz! You are truly an integral part of the team!

Seasonal Affective Disorder (SAD) is a form of recurrent depression that appears in predictable seasonal patterns. While most people associate SAD with the short, dark days of winter, or Winter Blues, a lesser known variant occurs in summer. Though both types share a cyclical nature and similar core symptoms, their triggers and physiological mechanisms differ in important ways.

Both summertime and wintertime SAD involve disruptions to mood regulation linked to seasonal changes. Individuals with either form may experience persistent sadness, low motivation, sleep disturbances, and difficulty concentrating. In both cases, the body’s internal clock plays a key role revolving around light exposure. Serotonin, melatonin, and circadian rhythm imbalances are common underlying factors. Additionally, both types often exhibit yearly consistency, with symptoms returning in the same season unless treated. The most significant difference is the environmental trigger.

Wintertime SAD, the more common type, is associated with reduced daylight, which can lower serotonin levels and increase melatonin production, leading to fatigue and low mood. Symptoms often include oversleeping, increased appetites, especially for carbohydrates and easy weight gain. Treatment frequently involves light therapy, which compensates for decreased natural light. Summertime SAD, by contrast, appears to be triggered by heat, humidity, excessive sunlight, or disruptions in daily routine caused by longer days. Symptoms tend to be almost the mirror image of winter SAD: insomnia instead of oversleeping, decreased appetite rather than cravings, and feelings of agitation or anxiety rather than sluggishness. Summer SAD is less common and may require different strategies, such as cooling environments, maintaining consistent sleep schedules, and sometimes limiting light exposure.

Despite their differences, both forms of SAD are legitimate, diagnosable conditions that can significantly impact quality of life. Understanding how each type manifests allows individuals and clinicians to tailor treatments to the season specific challenges and maintain emotional well being year round.
