Join Stillwater! We are hiring for a Registered Nurse Hospice Care Case Manager to serve hospice patients in Warsaw/Kosciusko County.
This position qualifies for a $2,250.00 Sign-on bonus. $750.00 after 90 days of employment. No Strings attached! Then $ 1,500.00 is awarded after 1 Year of employment, with the understanding that 12 additional months are required to retain the second installment of the bonus.
This is a full-time, on-site Registered Nurse/Hospice Care Case Manager position based in Warsaw, IN. The role involves managing and coordinating patient care plans, providing nursing care, and offering support to ensure the comfort and well-being of hospice patients. As a case manager, you will collaborate with an interdisciplinary team to deliver personalized medical care, guide families through the process, and provide critical care and emotional support. The role is essential to delivering exceptional and compassionate end-of-life care.
Qualifications
• Strong skills in Nursing and Patient Care to assess, plan, and implement patient care effectively.
• Expertise in Medicine and Critical Care Medicine to provide high-quality hospice care and address complex medical needs.
• Proficiency in Nursing Management to oversee and coordinate care plans with a multidisciplinary team.
• Excellent communication and interpersonal skills to support patient needs and provide guidance to families.
• Valid Registered Nurse (RN) license in the state of Indiana and any required certifications for hospice care.
• Experience in a hospice or palliative care setting is preferred but not mandatory.
• Compassionate approach and commitment to delivering patient-centered end-of-life care.

A century of commitment

The seeds of Visiting Nurse were planted in 1888, when women of the growing city of Fort Wayne, Indiana, banded together to deliver food and medicine to the “sick poor.” On March 1, 1900, Visiting Nurse was incorporated and hired its first paid nurse. Visiting Nurse’s mission is to provide compassionate care to alleviate suffering and ensure quality of life for those affected by serious illness through our hospice and palliative care services. Visiting Nurse cares for those who go on living, with grief support services available at no charge for any adult who has experienced the loss of a loved one. Hospice involvement is not required. We serve these Indiana counties: Adams, Allen, DeKalb, Huntington, Noble, Wabash, Wells and Whitley.
