Grey Stone Health and Rehabilitation Center provides a rehabilitation, skilled nursing and long-term care in a beautiful, brand new, state-of-the-art facility conveniently located near I-69 and Parkview Hospital. Our residents enjoy all that Fort Wayne has to offer, including zoo outings, popular restaurants, baseball and hockey games, bowling and more.

Grey Stone’s mission is to provide the highest quality care. Our physical, occupational, and speech therapists help our rehabilitation patients reach their highest physical and mental potential. Grey Stone's modern features and amenities, including spacious private rooms and bathrooms, will make your rehab experience a great one.

We believe that staying active reduces the effects of growing older, so residents are encouraged to be as active as they can be with activities that focus on exercise, fellowship and fun.
