Fort Worth Transitional Care Center

We provide short and long-term skilled nursing care that helps older adults achieve true wellness. We are a non-profit, and that’s an important difference.

Our focus is on strengthening the families and communities we care for. Post-operative care, Wound Care, IV Therapy, and an in-house rehabilitation team are some services that make our facility different. Unlike others who contract their therapists, we have an in-house therapy team. This means our goal is to have patients and residents see the same occupational, speech, or physical therapists daily, which builds a strong line of trust and rapport.

💙🎁 Our facility was proud to team up with Unbound Now to support their facility, "The Underground." This 24/7 drop-in center in serves youth ages 10–22, providing a safe space and critical resources. The Underground helps identify survivors and those at high risk of trafficking early, helping prevent further victimization.

This holiday season, our team members filled THREE carts with blankets, coats, gloves, and hats to support these incredible young people. We're grateful to be part of such a caring ! ✨

Las Posadas has begun! ✨🎄 Today begins a beautiful tradition cherished across Mexico and Latin America, honoring Mary and Joseph’s journey in search of shelter before the birth of Jesus.

From candlelit processions and heartfelt villancicos to sharing tamales, buñuelos, and piñatas filled with joy, Las Posadas brings families and communities together in a spirit of hope, hospitality, and celebration. To all who celebrate, may these nine nights be filled with light, tradition, and togetherness. 🌟
