Concho Hearts Hospice Fort Worth is a locally owned hospice company specializing in attentive, compassionate and thoughtful palliative care to patients and families in the Mansfield area and beyond. We take pride in providing a safe, warm, and attentive environment for those entrusting their care to us. Our DON and nursing staff have decades of experience in providing top notch hospice care in Tarrant and surrounding counties. In addition to our office, Concho Hearts also has offices all over Texas! Meet Our Staff:

Monica Merritt, RN is our awesome Director of Nursing. She provides oversight of weekly nursing visits, maintains quality assurance, organizes continuing education for staff, and assures program integrity. Our RNs bring a passion for hospice service to the bedside with compassion, warmth and knowledge. They have extensive hospice experience and hearts for service. It is a blessing to have their experience and truly caring hospice perspectives on this team. Erika Clausen is our amazing LVN who attends to her patients with a true passion for hospice nursing. Her compassion and dedication is unrivaled. Our on-call team is exceptional and is comprised of tenured hospice nurses. Katie, Shonda and Dusty bring expertise, compassion and extraordinary attention to detail to all aspects of hospice nursing. Their kind and caring nature and love for hospice make them an invaluable resource to our clients for both day and evening calls. Tosha Williams, Jasmine Smith, and Princess Richardson are our phenomenal aides and are usually the patient's favorite part of the team. Their care and attention to detail keep patients feeling their best while maintaining the utmost dignity of the patient. Millie Hunt is our Marketing Manager and Community Liaison. She provides consultations to families, acts as a First Contact and has a plethora of information to help families make the best decisions for them when facing terminal illness. She is outstanding at explaining how hospice can work for you and your family. Her gentle approach and calm demeanor allow families to consider their options in a relaxed environment. Karen Taylor is our Intake Coordinator/Office Manager. She wears many hats and wears them well. She expertly manages the details of the referral/admission processes as well as some major compliance aspects. She has many years of hospice intake and office experience keeping all things running with precision behind the scenes. Our phenomenal Chaplains are available to staff and clients to provide support, comfort and care for all spiritual issues during a client's stay on hospice and after the journey has ended, for the family. They are friendly, warm, non-judgmental personalities, providing a non denominational listening ear and encouragement/support to our clients and their families (as well as our staff). Our Social Work teams are attentive to the needs of our clients and their loved ones. They can provide information and resources to help meet the needs of our families. They can also help with official forms and paperwork, end of life planning, facility placement and other questions you may have. Our distinguished Medical Director, Dr. Elizabethe Carter, manages symptoms such as pain, shortness of breath, nausea and anxiety and is available to our nursing staff 24/7 to be sure all needs are met for our clients and their families. When you trust your family with us, you're part of ours. The holidays look different when someone you love is facing a serious illness—or when their absence is still achingly new.

While the world around you celebrates, you may be carrying what researchers call "anticipatory grief"—mourning before a loss occurs—or navigating the tender terrain of your first holidays without someone who made them meaningful. The expectations we place on ourselves during this season—the perfect gatherings, the joy, the traditions—can feel impossible when your heart is somewhere else entirely.

Here's what we know: grief doesn't follow a calendar, and it certainly doesn't take a holiday break. Holidays magnify loss because they're built around relationships and shared memories. You may experience what's called "disenfranchised grief"—grief that isn't fully acknowledged by others, creating pressure to appear cheerful so you don't disrupt celebrations.

After 21 years in hospice, Millie knows what matters most: the people beside you doing the work and the difference you make to families in your care. Millie is known for her compassion and consistency. As a child, the opportunities she was given to help care for the elderly shaped who she became. Now, she brings compassion, consistency, and genuine presence to every patient in their end of life journey. When she's not with patients, you'll find Millie at concerts, taking walks, spending time with her grandkids, or talking with God. Millie's dream is to use her role, leadership expertise and education to do more of His will.
