rtNOW incoming call at 08:32PM from RN at small community hospital ED caring for ***** ***** DOB 01/01/1967 MRN ********. Patient came in for shortness of breath due to CHF and fluid retention. V60 BIPAP was started with the following settings on St mode 10 10/6 30% with improved respiratory rate from 40's down to mid 20's saturation at 99%. Patient respiratory rate on V60 after patient was given Dilaudid and Lasix was 18, VT 547, Minute ventilation 11.3 and alarms set appropriately. VBG as patient was getting placed on V60 was the following 7.38/39/76/22.6/-2.3. I suggested getting a repeat ABG/Vbg in the next hour and I will call back within that time. Nothing else was needed from RT and advised to call if something should arise. rtNOW call ended at 08:40PM.

rtNOW follow-up call at 09:54PM to talk to RN caring for ***** ***** DOB 01/01/1967 MRN ********. VBG/ABG is still pending and will call back for recommendation if it has abnormal results. RN is going to be drawing labs shortly, but was busy with other patient. Patient will be admitted to the Med/Surg floor and staying at small community hospital. Instructed RN to call when transfer to floor is complete or if anything else should change. rtNOW call ended at 09:55PM.

Here is this week's actual inpatient encounter note. This is what we do!

rtNOW incoming call at 148pm from Med/Surg RN at small community hospital to have an assessment completed on patient ***** ******* DOB: 1/1/1939 MRN ********** . Upon assessment, patient was using 3 lpm O2 via Nasal Cannula with O2 saturation of 96%, RR-16-20, getting nebulizers frequently. Advised to get an ABG/VBG to see if patient was retaining any CO2. Advised that if patient started to have increased work of breathing to start on Airvo 25 lpm and 35% O2 and titrate to maintain desired saturation OR initiate BiPAP depending on result of blood gas. rtNOW incoming call @10:02 am from ED RN ***** at ***** **** Hospital. Patient *********, DOB: 1/1/1938*,MR # ********. Patient arrived at hospital with shortness of breath and increased work of breathing. Patient wears home oxygen but the oxygen sats on arrival were in the 80's. Placed on NIV-ST on Hamilton C1. P-inspiratory of 12, Peep +7 30% FiO2. Measured values on these settings: PIP 19,RR 28,VT:613,MV:16.8. Duonebs, solumedrol, and magnesium given by RN. Plan: Continue on current settings and evaluate in 1 hour. Call ended at 10:21 am.

rtNow called back RN ***** @ 10:56 am to check on patient status. Patient doing well on current NIV settings on Hamilton c-1. O2 sats at 94% and respiratory rate has normalized. No video call needed at this time. Plan: Will continue on current settings and wean as able. RN will call back as needed. Call ended at 10:57 am.
