Six states, including TX, will be implementing prior authorization procedures in 2026 to help limit fraud, waste and abuse for 17 medical services. This affects those in Original Medicare that have Medigap plans.

The Centers for Medicare and Medicaid Services (CMS) will implement prior authorization requirements for certain traditional fee-for-service Medicare services in six states starting next year.
