I don’t use textured breast implants.

Yes, the risk of BIA-ALCL is exceedingly rare—but rare isn’t the same as impossible. I’ve personally taken care of a patient who was referred to me that was ultimately diagnosed with this condition, and once you see it firsthand, the risk stops being theoretical.

For me, any potential benefit of a textured implant just isn’t worth even a small, avoidable risk when safer alternatives are available.

I’ll be sharing more details and some photos of this patient later this week—not to scare anyone, but to educate and be transparent about why I make the choices I do as a surgeon.

I’m often asked about drainless tummy tucks. My approach is selective. If liposuction is performed, I typically use drains. Why? Because I prioritize minimizing seroma risk over trends. Surgical decisions should be based on anatomy and safety, not marketing.

Today I’m removing a Silimed Quartzo gluteal silicone implant — a type not FDA-approved in the U.S. and more commonly placed by surgeons abroad.

This patient had her implants for over 10 years, and one of them began causing worsening pain, which led her to seek removal.

A primary goal during explantation of gluteal implants is minimizing the risk of seroma formation. I use quilting sutures, drains, and compression to help with this.
