Friendly reminder:
When you book an appointment you are sent a confirmation text and email. This confirmation also includes the link for your forms that need to be done prior to your appointment and your deposit. If your deposit isn’t paid within 24 hours the system automatically cancels your appointment. I send one reminder text about the deposit after that if you do not pay your deposit and your appointment gets cancelled you will need to reach out to me to rebook.
Forms have to be filled out every single appointment. This helps to ensure all your current info is up to date and so I can provide the best care possible. It also allows me time to contact you if I have any questions or if there are any contraindications that would keep me from being able to work on you. The forms and deposit take less than 5 minutes to do.
Please remember the forms are for your health and safety and to ensure the best care possible.
