CCACC Health Center 泛亞門診 CCACC Health provides primary care, preventive care, health-related services, and welfare applications for uninsured low-income residents of Montgomery County.

Community health clinic, serving Montgomery County, MD since 2003. Services:
- Primary Care
Diagnosis and treatment of acute and chronic illnesses (i.e. diabetes, hypertension, and asthma).
- Referral Services:
Patient referrals to connected facilities for diagnostic imaging at a discounted rate. Patient referrals to specialists and health management plans for follow-up care.
- Case Management
- Education and Outreach

Requirements:
- Montgomery County resident
- Uninsured
- Income below 250% Federal poverty level
- Age 18+

Bilingual Staff (Chinese).

March is Developmental Disabilities Awareness Month 💛 a time to celebrate neurodiversity and remind us that growth happens when we lead with understanding and compassion. 🌼

At CCACC Health, we’re proud to support children and families through our PROMISE Program — Promoting Meaningful Interaction and Social Engagement. 🌈 PROMISE offers a warm, supportive environment where children with special needs can build confidence, connect with peers, and engage in meaningful, adaptive activities to help them build essential skills. 🌱

PROMISE is a collaboration between the of Maryland Special Education Program and CCACC, led by professionals. Together, we create a safe space where every child is valued and encouraged to thrive. 🤗

Good nutrition doesn’t have to be complicated — small, consistent choices can make a big difference.

Try these 5 tips for better nutrition and a healthier you 以下是 5 個營養小技巧,幫您吃得更健康:

🥦 Prioritize Fiber: Fiber supports digestion, heart healthm and blood sugar balance. Try adding beans, broccoli, apples, whole grains, or sweet potato into your diet.

Colorectal cancer is one of the most preventable cancers — screening is the key. Early detection can save lives, especially because symptoms often don’t appear until the disease has progressed. This month, we’re encouraging our community to take charge of their health.
