To Our Patients,
Thank you for allowing our practice to care for you over the past 13 years. It has been our privilege and honor to serve the Galveston Community and your family. Following the untimely and tragic death our beloved Nancy L. Hughes, M.D., on March 18, 2022, we continued to see our patients with the assistance of Joie Brownlee, our nurse practitioner, and Dr James King, a physician from UTMB Health. It is now time to close the practice and transition our patients to providers at UTMB Health.
Whether you live on Galveston Island or on the mainland, UTMB Health offers several locations for primary care throughout Galveston County and the Clear Lake area. To facilitate your transition of care to UTMB Health, we have dedicated phone lines for you to call if you would like a primary care provider off the island (409-986-9686) or on the island (409-747-1883).
Please be advised that our office will no longer see patients after Friday, October 21, 2022, so we encourage you to choose a new provider soon. Phones will continue to be answered and medical records will be available at our office until Friday, November 4, 2022. After this time, the practice will be closed.
We have arranged for UTMB Health to house medical records for all of our patients after the practice closes. If you would like to seek care outside of UTMB Health, our office can provide your medical records until November 4th. After that date, you may contact the UTMB Health Information Management Department at 409-772-1965 to transfer your records to another provider. The information Management Department will email or fax the proper authorization form to you, or you may pick up the form at their office.
