SoHum Health is a California Special Healthcare District that operates Jerold Phelps Community Hospital, Southern Humboldt Community Clinic, Southern Humboldt Family Resource Center, Garberville Pharmacy, Fortuna Optometry, and Garberville Optometry. Dating back to 1949, healthcare services have been available at the current site of the Southern Humboldt Community Healthcare District (SHCHD) in Garberville, California. SHCHD currently includes Jerold Phelps Community Hospital with its attached skilled nursing unit; Southern Humboldt Community Clinic, a certified rural health clinic; Southern Humboldt Family Resource Center at Redway Elementary; and Garberville Pharmacy, a full-service retail pharmacy located at 286 Sprowel Creek Road. Our service area is approximately 775 square miles and includes the communities of Alderpoint, Blocksburg, Garberville, Harris, Honeydew, Miranda, Meyers Flat, Leggett, Petrolia, Phillipsville, Piercy, Redcrest, Redway, Shelter Cover, Weott, Whitethorn, and Zenia. With strong community support, including a supplemental property tax, SHCHD continues its work to assure quality healthcare for residents and visitors alike. The hospital received Critical Access Hospital (CAH) designation status in March 2002, helping position SHCHD for a more sustainable future. Direct services include an acute care hospital, 24/7 emergency room, laboratory, radiology, physical therapy, occupational therapy, mammography, x-ray, home health/visiting nurse program, swing-bed, visiting specialists, skilled nursing, a rural health clinic, the Family Resource Center, and retail pharmacy services. Katelyn Pagano, OTR/L believes that healing happens through connection, creativity, and meaningful activity. Since earning her Master's in Occupational Therapy from Colorado State University in 2015, Katelyn has practiced in diverse clinical settings - ICU and acute care, skilled nursing, lymphedema management, memory care and cognitive therapy, and outpatient pediatrics. Passionate about rural healthcare, Katelyn values building relationships and supporting community members across the lifespan. She has a particular interest in supporting children with neurodevelopmental diagnoses and their families, providing education on adaptive mobility and technology, and supporting folks with chronic conditions and functional changes - always with a focus on dignity, independence, and quality of life.
