CCNC brings whole-person care management to all 100 counties across North Carolina. Our model focuses on solving problems close to home and spreading effective solutions statewide.

Care Manager Terri White, RN, CCM, recently worked with a member who had relocated to North Carolina and was living without electricity, food, transportation, or medical support. Terri arranged his first CCPN primary care visit, coordinated dental and vision care, restored basic utilities, and connected him to daily nutrition and transportation services.

With regular primary care and home-delivery pharmacy services now established, he is receiving the ongoing support he needs.

A new resource from the American Academy of Family Physicians (AAFP) is now available to help clinicians and practices support patients facing social needs. The Neighborhood Navigator lists more than 40,000 agencies that provide food, housing, transportation, and related services.

Some patients may still feel the effects of delays or changes to SNAP benefits after the recent federal shutdown, and this tool can make it easier to connect them with immediate help. A video on the site walks through how to use the resource effectively: https://bit.ly/43PDpgq

To support practices affected by Medicaid rate cuts, CCNC is advancing 2024 earnings across all five prepaid health plans. Although these health plans have not all provided final settlements, we are confident in the numbers and hope that this will provide immediate financial assistance for practices through Quality Incentive payments and shared savings.

In total, $19 million in new funds are being distributed to CCPN Advanced Practice Medical Home practices for their strong 2024 performance, bringing this year’s total to over $24 million in payments.

CCNC and CCPN will continue to stand with our members and advocate for North Carolina’s independent primary care providers.

Congratulations to Seven Peaks Family Medicine in West Jefferson for being featured in the October issue of Business North Carolina !

Drs. Victoria Knox, Jason Karimy, and Jessica Stevens met during residency in Boone and decided to open a new practice together, choosing rural Ashe County over big-city medicine.

With guidance from CCNC, they secured financing, developed a business plan, and built a 5,000-square-foot clinic. The team is now providing full-spectrum, lifelong care for families in their community.

November is National Diabetes Awareness Month, and we’re celebrating the incredible work of our Advanced Primary Care Management (APCM) Team.

APCM is a CCNC program that gives practices access to a full care team, including a clinical pharmacist, registered nurse, and health navigators who work one on one with patients to manage diabetes and other chronic conditions.

It’s all about improving outcomes, reducing barriers, and delivering patient-centered care across North Carolina.

New data brief from CCNC: “Effectiveness of Care Management for Engaging Patients with Their Medical Homes”

This brief examines how care management helps Medicaid patients reconnect with their primary care providers. Findings show:
• Patients receiving care management were much more likely to schedule a visit.
• The effect grew stronger the longer it had been since their last visit – up to a 68% increase for those who hadn’t seen their doctor in 4+ years.
• Non-Hispanic people of color and Medicaid expansion enrollees were less likely to engage, pointing to opportunities for more focused outreach.

The takeaway: Care management has the greatest impact among patients who are hardest to reach, and that’s where it matters most.

The platform also navigates patients to high-quality, low-cost specialists, fast tracks urgent needs, supports chronic care management, and provides a simple referral dashboard across 30+ specialties.

Is your CCPN practice on track to reach 100 PEI points before the end of the year?

The Practice Engagement Index (PEI) tracks activities aligned with the Medicaid performance year, Jan. 1 through Dec. 31. To qualify for the maximum financial incentives, practices must earn 100 points and submit documentation of completed activities before the end of the year.

On Nov. 20, join NC Department of Health and Human Services for the next Back Porch Chat moderated by Dr. Janelle White, chief medical officer at NC Medicaid.

This session will focus on the Children and Families Specialty Plan, a new statewide managed care plan launching on Dec. 1 under Healthy Blue Care Together. The plan is designed to ensure coordinated care for children, youth, and young adults currently or formerly in foster care.

Representatives from Healthy Blue Care Together will share implementation updates and answer your questions. The discussion will also cover H.R.1 and what it means for Medicaid in North Carolina.

We’re celebrating a successful year with Invision Diagnostics. The Invision Mammogram Bus visited 14 CCPN practice sites and screened hundreds of patients, helping close critical care gaps for Medicare and Commercial patients.

Thank you to the adult Blue Premier practices that scheduled the bus. Our custom T-shirts were a hit and made mammogram event days feel unified and festive.
