Reconstructing the nose after removal is one of the most delicate and rewarding challenges in facial surgery - particularly after where the goal is complete cancer removal while preserving as much healthy tissue as possible.
One powerful option for nasal reconstruction is the Peng flap, also known as an anchor flap. This versatile, single-stage flap is particularly well suited for medium-to-large defects, most commonly on the central nasal tip - one of the most visible and structurally complex areas of the face.
The Peng flap is a modification of a linear advancement flap, but with an important innovation: it features two “rotating arms” that advance from the cheeks and nasal sidewalls toward the defect. These arms naturally follow the curved anatomy of the nose, allowing the tissue to rotate and advance smoothly rather than pulling straight across. This design reduces tension, improves flap survival, and helps maintain the nose’s natural contours.
One of the greatest strengths of the Peng flap is its excellent color and texture match. By borrowing tissue from immediately adjacent areas, the reconstruction blends seamlessly with the surrounding skin. Just as importantly, the helps recreate the nose’s three-dimensional shape, preserving the natural convexity of the nasal tip rather than flattening it.
From an aesthetic standpoint, the Peng flap also fits beautifully into the nose’s existing cosmetic subunit lines, allowing scars to be placed where they are less noticeable and more likely to heal discreetly over time. The result is a reconstruction that looks natural- not tight, not distorted, and not “operated on.”
While no single technique is right for every patient or every defect, the Peng flap is often a preferred choice for central nasal tip repairs because it balances form, function, and long-term appearance so well. It’s a perfect example of how thoughtful surgical design can restore not just skin, but confidence.
isn’t just about closing a wound—it’s about respecting anatomy, honoring aesthetics, and helping patients feel like themselves again.
