PhyNet is pleased to welcome Kalana Sanders, FNP-C, to the PhyNet Hugman-Kent care team. Kalana is a certified family nurse practitioner at Hugman-Kent Clinic in Gladewater, Texas, where she provides care to pediatric patients (ages 4 and above) and senior adults.

Kalana began her healthcare journey in 2018 as a registered nurse after earning her Bachelor of Science in Nursing from The University of Texas at Tyler. During her nursing career, she received the prestigious Daisy Award three times, recognizing her compassionate bedside manner, exceptional communication skills, and unwavering dedication to patient care.

Driven by a passion for delivering well-rounded, patient-centered care, Kalana returned to The University of Texas at Tyler to pursue her Master of Science in Nursing as a Family Nurse Practitioner, graduating in 2023. She is experienced in managing both chronic and acute conditions, with a strong emphasis on preventive care.

Kalana is committed to building meaningful relationships with her patients, taking the time to educate, empower, and guide them in making informed decisions about their health. Her approach to care is personalized, evidence-based, and rooted in trust.

Outside of work, Kalana enjoys spending quality time with family and friends, traveling, attending concerts, reading, and cooking.

Medical Assistant (MA) – Care Coordination
(Not Remote–Working with Virtual CareTeam in Longview, TX)

Join Our Team! PhyNet is seeking an MA to work with the care team from a centralized office setting, focusing on remote patient support and care coordination rather than in-person clinical tasks. The role helps ensure ongoing engagement for patients with chronic conditions.

CORE ROLES:
Perform follow-up calls for routine check-ins, medication adherence, and care plan compliance.
Coordinate appointments, referrals, and diagnostic services.
Document interactions and care updates in the EHR.
Assist with tracking preventive care and managing care gap lists.
Communicate with patients and providers to support continuity of care.

CERTIFICATION / EXPERIENCE:
Certified MA preferred; BLS
Prior experience in remote patient coordination or outpatient care setting is a plus.
