Glastonbury Center for Health & Rehabilitation Offering short-term rehab, dementia care, long-term and respite care, and hospice services. The ultimate goal of our approach is to restore you to health.

About you
Our center provides guests with a variety of individualized services and programming, whether they are with us for a short stay following a hospitalization or surgery, or they will call us their new home for long-term nursing care. Our dedicated staff is focused on delivering the highest quality of services, with a common goal of helping guests become better, brighter and stronger versions of themselves.

• Passport TM Approach •
Our Passport TM Approach is a natural progression of our services, helping our short-term rehabilitation patients return to the best quality of life possible, as quickly as possible. The approach we take optimizes recovery through individualized occupational, physical and speech language therapy plans with goals tailored to their specific needs and preferences. We employ trained specialists equipped with a diverse set of skills who are ready to field the most unique challenges in your rehabilitation process. We understand that every person under our care has distinct requirements for therapy. Your individual rehabilitation is tailored to a series of realistic, short-term achievements based on getting you home. We encourage you to meet with our interdisciplinary team of specialists to discuss your goals and expectations prior to your stay. With a patient-centered approach to care, our program is designed to provide you with the skills and abilities you need to live a healthy and fulfilling life.

• Long-Term Skilled Nursing Care •
All National Health Care Associates affiliated centers also specialize in delivering quality long-term care to residents. The team develops a comprehensive care plan which includes all medical, nursing, rehabilitation, nutritional, psychosocial, and therapeutic needs. Each resident is also evaluated upon admission by our interdisciplinary team, based on need for rehabilitative services in physical, occupational and/or speech therapy. Thereafter, a team of therapists carry out restorative or maintenance therapies in conjunction with the resident’s plan of care. Our goal is to assist each resident with the means to maintain his/her optimum level of functioning.

• Alzheimer/Dementia Special Care •
This program is designed to meet the special needs of Alzheimer’s/Dementia residents in a safe, caring and secure environment. Our goal is to help residents with Alzheimer’s or related dementias maintain their current level of functioning.

Together, We Walked for a World Without Alzheimer’s

During the month of October, every one of our Connecticut Centers proudly joined the Walk to End Alzheimer’s, with team members representing at seven walks across the state. Our dedicated teams came together with one goal — to honor those living with Alzheimer’s and to help find a cure.

We’re incredibly proud to share that together, we raised over $26,000 (and counting!) with 123 participants across our Connecticut Centers. Every dollar raised helps fund critical Alzheimer’s research, care, and support programs.

A heartfelt thank-you to every team, every walker, and every donor who helped make this impact possible. Your compassion and dedication truly make a difference.

This year, every one of our Connecticut Centers proudly joined the Walk to End Alzheimer’s, with team members representing at seven events across the state. Each step, smile, and show of support brought us closer to a world without Alzheimer’s.

Together, our Centers have raised an incredible $26,000 (and counting) — a true testament to the compassion, unity, and dedication that define our National Health Care Associates family.

Every 40 seconds, someone in the U.S. experiences a stroke—and it can happen anywhere. That’s why recognizing the warning signs and acting quickly is so important.

Remember B.E. F.A.S.T.—the signs of a stroke:
-Balance Unsteady
-Eyesight Changes
-Face Drooping
-Arm Weakness
-Speech Difficulty
-Time to call 911

The sooner a stroke is treated, the better the chances for recovery. Patients who receive clot-busting medication or stent procedures within the first critical minutes are three times more likely to recover with little or no disability.

Stroke is largely treatable—but only if we act fast. Learn the signs, share the knowledge, and be prepared.
