 We are getting a lot of questions about the new hybrid COVID booster. The answers are personalized to the individual. Some principles that I follow are:

1. Stick with the same brand that you started with (Pfizer or Moderna).
2. Receive the vaccine where side effects can be managed (Urgent care or doctor’s office)
3. Wait two months from your last booster
4. If you had COVID 19 disease, then you should wait at least 3 months. The CDC said earlier that those who had the disease are immune for 6 months.
5. Never take two vaccines in the same day. If you do this and have side effects, then you will not know which vaccine caused the problem.
6. Wait until October to get a flu vaccine. It produces maximum immunity at 12 weeks. Influenza typically appears in late January/early February.
7. Our office and some pharmacies upload vaccine information to the Virginia vaccine database. Employers and grocery stores do not. It is important to have long term records of your immunizations that any physician in the state can access.

Daniel joyfully accepted the offer and his on his way to earning an M.D. I am proud of this bright, young man and honored to have been his first mentor on this noble path.

Two of my patients, who went to the Final Four in New Orleans last week, have COVID-19. It looks like that was a super-spreader event. COVID is on the rise again. If you have symptoms, please get tested. If you are eligible for a vaccine booster, please get it.

We are seeing a decline in new COVID cases and hospitalizations. I believe this is due to the prolific spread of the Omicron variant of the COVID19 virus. By spring, I believe that everyone in the USA will have either had the virus, had a vaccine, or both. Americans will likely achieve the “Herd Immunity” which has eluded us for the last two years.

The herd will remain immune until another variant reaches our shores from abroad. The original COVID virus came from China. The Delta variant came from India, and the Omicron from South Africa.

In order to preserve the immunity of our people, I believe that we should ban all international travel. Business travel has largely been replaced by Zoom and similar applications. That leaves only tourism. Stopping international tourism is a small price to pay to keep Americans healthy and end this plague, in my humble opinion.

If our borders remain open, then we invite something even worse to infect our people.

Fish oil supplements have long been marketed for prevention of heart attacks and stroke. Peer-reviewed medical studies do not support these claims. Because these products are classified as “dietary supplements”, the Food and Drug Administration (FDA) does not stop their sale. Physicians have been warning patients for years that fish oil supplements are a waste of money. I recently learned that fish oil is derived from a fish called the menhaden. It is too small for humans to eat but is rich in oil. The menhaden is a “keystone species” in the Chesapeake Bay. Larger fish, migratory birds, and marine mammals like otters depend on menhaden. Menhaden eat plankton by filter feeding. They purify the Bay and the ocean and prevent catastrophic algal blooms. The fish oil industry is endangering menhaden populations in the Atlantic. So now there are two reasons NOT to take fish oil supplements. 1. They are a waste of money. 2. They harm the Chesapeake Bay.

A long time patient (let’s call her Greta) called me at the end of September. Greta comes from a large family and has shepherded many of her siblings through illness and death. She has a golden heart. She was distraught with grief because her 46 year-old niece was hospitalized with COVID. I comforted her as best as I could over the phone. Thursday she called again because her niece had died. She was in such shock that she hardly spoke. It broke my heart. PLEASE GET VACCINATED against COVID-19

Update on the post from August 15th about Bob:
Bob came back for his follow-up appointment. Thankfully, he is off of supplemental oxygen. However, he is still not able to work because he becomes short of breath. I approved him for one month of short term disability, but his insurance company approved only one week. He has missed FIVE WEEKS of pay. He is going to the lung specialist next week. PLEASE GET VACCINATED AGAINST COVID 19!

On Friday the 13th, my first patient of the morning was a gentleman whom I shall call Bob. He was brought to me by his girlfriend who has been a long-time patient. Bob is 52 with no medical history other than refusing COVID vaccination. He started to feel ill on July 20 and had to stop working. His breathing became worse and he finally went to the hospital where he was admitted with COVID pneumonia affecting two lung lobes. His condition worsened despite remdesivir treatment until the doctors had to put him on a respirator. His girlfriend became his power of attorney in case life support had to be withdrawn. At that point, eight of his friends who had refused vaccination got vaccinated. (His girlfriend had been vaccinated months ago). Bob's condition improved and he was able to leave the hospital a week later. However, he has to wear oxygen continuously and is not ready to go back to work. The hospital doctors told him that his lungs will never be normal again. I saw another patient last Friday with a similar story because she also refused to get vaccinated. The hospitals in the Richmond area are diverting patients away from their emergency rooms because they are overwhelmed by COVID patients. PLEASE GET YOUR COVID 19 VACCINE. I am puzzled why people still refuse to get vaccinated.

As many people are now vaccinated, wearing masks in public seems to be fading. Please remember that masks not only protect you from coronaviruses but also from other respiratory viruses. Most people do not realize that influenza was almost nonexistent this past winter. This is partially due to the fact that people were wearing masks. The president of one of our local hospitals told me that in January 2021 they had only 11 cases of influenza in the hospital. In a typical January, there are 400 cases. so keep wearing your masks in public as much as possible. I intend to keep wearing my mask when seeing patients in my office.

I am accepting applications for a new scribe. Over the last eight years, I have developed a systematic method of teaching my scribes skills needed to practice high-quality medicine. My previous scribes have said working with me was the best hands-on training they got before medical school. The position is full-time with benefits. I require a one-year commitment.

We have Johnson and Johnson COVID vaccine available in our office. This is the one that requires only one dose. While there is a one in a million chance of blood clots forming in women, there are no reported significant side effects in men. Patients have complimented our staff on their efficiency and courtesy in the vaccine clinic. The process is usually less than 45 minutes. If you are a college student home for the summer, feel free to call our office Monday for a vaccine appointment. Vaccination may be required for summer jobs and to return to school. My son was vaccinated yesterday in our office.
