Hair thinning can appear near the incision sites because the tissues are lifted and secured under tension. When hair follicles are stressed or their blood supply is temporarily disrupted, “shock hair loss” can occur.

But important is to consider for each tox is The difference in depth of injections, how much it spreads, how deep you inject. For results - factors such as skin tone, skin laxity, age, gender, muscle unique anatomy, lifestyle, medications and etc play great role.

The Depressor Anguli Oris (DAO) muscle plays a key role in controlling lower facial expression. Its primary function is to pull the corners of the mouth downward, contributing to expressions of sadness or disapproval. Anatomically, the DAO originates from the oblique line of the mandible and inserts into the modiolus—the fibromuscular hub at the corner of the mouth.

Because of its downward pull, overactivity of the DAO can create a persistently downturned or “sad” appearance at rest. Neurotoxin treatment can help relax this muscle, allowing the corners of the mouth to lift slightly and restoring a more positive expression.

However, precise anatomical knowledge is essential. The DAO lies in close proximity to the Depressor Labii Inferioris (DLI), a muscle responsible for lowering the bottom lip. Misplaced injections can inadvertently affect the DLI, leading to asymmetry or difficulty in lower lip movement.

Before performing injections, it’s important to palpate and identify both muscles accurately. Asking the patient to actively depress the corners of the mouth can help visualize DAO contraction, while having them pull the lower lip downward isolates the DLI. This differentiation ensures targeted toxin placement, minimizes unwanted diffusion, and reduces the risk of complications such as lip droop or uneven smile.
