More often, it is barrier disruption.

Jowls are one of the most misunderstood, and most frustrating, changes we see in the face.

They are not simply “loose skin.” A jowl is a vertical fold that forms where the jawline meets the chin, created by a very specific combination of structural changes happening beneath the surface.

With time, the skin loses collagen and elastin, reducing its ability to resist gravity. At the same time, the underlying bone subtly remodels — particularly along the gonial angle and prejowl region. As this support system weakens, the superior and inferior jowl fat pads begin to shift forward.

The fat pad descends and folds over itself, influenced by the muscular attachments of the lower face. What appears externally as a small shadow or heaviness is actually the visible result of changes in skin, fat, and bone working together.

This process can accelerate with significant or rapid weight loss as well, including what we frequently observe with GLP-1 related changes.

Why does this matter? Jowls do more than alter a contour — they alter expression.

They often create the unintended appearance of fatigue, sadness, or tension, even when you feel energized and vibrant. Many patients describe looking “older” or “heavier” despite feeling like themselves.

For some, surgical intervention such as a facelift offers the correction they want. For others, non-surgical approaches allow us to address the underlying mechanics more conservatively.

This may include:

• Restoring structural support along areas of volume loss (jawline, gonial angle, cheeks, preauricular space, and sometimes temples)
• Carefully disguising the fold when appropriate
• Improving tissue quality and reducing fat pad prominence with Matrix RF Microneedling

True correction is rarely about adding more — it is about restoring balance, support, and tension where the face has lost it.

The goal is never to look different. The goal is to look like you — just not tired, weighed down, or shadowed by structural change.
