Formerly Elant at Goshen, serving the Orange County community since the 1980’s, Sapphire Nursing and Rehab at Goshen is an Adult Day Care center and a 120-bed Skilled Nursing Facility. Adult Day Care services is a community based program designed for adults who need assistance, are disabled or chronically ill but prefer, and are safe, to live at home. Sapphire Nursing and Rehab at Goshen’s Day Care programs are under the direction of a Registered Nurse and delivers professional care in collaboration with an personalized interdisciplinary team. Both social and medical model day cares are offered at Sapphire Nursing and Rehab at Goshen. Our nursing home team is comprised of skilled, caring, highly trained staff to meet the specific needs of each of our residents. Sapphire Nursing and Rehab at Goshen’s skilled nursing services, both short-term and long-term, provide a variety of medical, clinical, social and psychological components to address the needs of individuals when the level of care needed can no longer be provided at home. An interdisciplinary team consisting of a nursing staff, dieticians, social workers, therapists (physical, occupational and speech) and activity leaders together work closely with our resident’s physician(s) to maximize independence and quality of life. Across the campus, our philosophy on care delivery, is to understand who our residents and registrants were prior to coming to us and empowering them through a variety of different approaches, to achieve their goals of care.

We held our first Annual Thanksgiving Day Parade at Sapphire Goshen, and it was such a hit! 🦃🎉

The parade traveled through the halls, rooms, dining areas, and lobby, bringing smiles to residents, families, and staff all along the way.
