In one of the largest outcome collections using Cox® Technic, patients with low back pain experienced relief in an average of 12 visits over 29 days.

For cervical spine conditions, the average was 13 visits.

These findings come from a large dataset of patient outcomes, offering a clearer picture of how structured spine care can progress over time.

While every patient and case is different, this kind of data highlights an important distinction:
𝗪𝗵𝗲𝗻 𝗰𝗮𝗿𝗲 𝗶𝘀 𝗽𝗿𝗼𝘁𝗼𝗰𝗼𝗹-𝗱𝗿𝗶𝘃𝗲𝗻 𝗮𝗻𝗱 𝗰𝗼𝗻𝘀𝗶𝘀𝘁𝗲𝗻𝘁, 𝗶𝘁 𝗯𝗲𝗰𝗼𝗺𝗲𝘀 𝗲𝗮𝘀𝗶𝗲𝗿 𝘁𝗼 𝘁𝗿𝗮𝗰𝗸 𝗰𝗵𝗮𝗻𝗴𝗲𝘀 𝗶𝗻 𝗽𝗮𝗶𝗻, 𝗳𝘂𝗻𝗰𝘁𝗶𝗼𝗻, 𝗮𝗻𝗱 𝘁𝗼𝗹𝗲𝗿𝗮𝗻𝗰𝗲 𝗳𝗿𝗼𝗺 𝘃𝗶𝘀𝗶𝘁 𝘁𝗼 𝘃𝗶𝘀𝗶𝘁.

That means both the provider and the patient can better understand what’s improving, and what needs to be adjusted.

Because in spine care, outcomes shouldn’t be left to guesswork.

When it comes to spine care, patients don’t always start by looking for a specific certification.

But over time, especially in more complex or persistent cases, the way they search can change.

Here are 5 reasons patients choose a Cox® Certified Physician:

1. 𝗔 𝗺𝗼𝗿𝗲 𝘀𝘁𝗿𝘂𝗰𝘁𝘂𝗿𝗲𝗱 𝗮𝗽𝗽𝗿𝗼𝗮𝗰𝗵 𝘁𝗼 𝗰𝗮𝗿𝗲.
Cox® Technic follows defined, protocol-driven methods rather than variation from visit to visit.

2. 𝗘𝘅𝗽𝗲𝗿𝗶𝗲𝗻𝗰𝗲 𝘄𝗶𝘁𝗵 𝗰𝗼𝗺𝗽𝗹𝗲𝘅 𝘀𝗽𝗶𝗻𝗲 𝗰𝗼𝗻𝗱𝗶𝘁𝗶𝗼𝗻𝘀.
Patients dealing with disc herniation, stenosis, or post-surgical pain may seek providers trained to manage these cases.

3. 𝗙𝗼𝗰𝘂𝘀 𝗼𝗻 𝗰𝗼𝗻𝘁𝗿𝗼𝗹𝗹𝗲𝗱, 𝗴𝗲𝗻𝘁𝗹𝗲 𝘁𝗿𝗲𝗮𝘁𝗺𝗲𝗻𝘁.
Care is typically delivered with an emphasis on precision and patient tolerance.

4. 𝗖𝗼𝗻𝘀𝗶𝘀𝘁𝗲𝗻𝗰𝘆 𝗶𝗻 𝗵𝗼𝘄 𝘁𝗿𝗲𝗮𝘁𝗺𝗲𝗻𝘁 𝗶𝘀 𝗱𝗲𝗹𝗶𝘃𝗲𝗿𝗲𝗱.
Protocol-based care can offer a more repeatable and predictable experience.

5. 𝗢𝗻𝗴𝗼𝗶𝗻𝗴 𝗰𝗲𝗿𝘁𝗶𝗳𝗶𝗰𝗮𝘁𝗶𝗼𝗻 𝗮𝗻𝗱 𝘁𝗿𝗮𝗶𝗻𝗶𝗻𝗴.
Active certification requires continued education, helping providers stay current with clinical protocols.

Patients don’t always begin here.
But many eventually start asking not just who can treat them, but how they are trained to do it.

In spinal care, consistency matters just as much as technique.

In this video, Dr. Steven Shoshany highlights how the Cox® 8 table with force technology package provides real-time, objective feedback during treatment, changing how providers assess and deliver care.

With measurable data such as:

- Y-axis force (decompression force)
- Hand-applied force
- Flexion angle of the table

…the provider can see exactly what is happening during treatment, not guess.

This level of feedback allows for:

- More precise decompression
- Consistent application of force
- Repeatable treatment protocols across providers
-Clear tracking of patient progression over time

Instead of relying only on feel, the provider can visualize and quantify how the spine is being unloaded.

For patients, this often translates into a different experience — one where the spine is gently decompressed, pressure is reduced, and the body can relax into the treatment.

For providers, it reinforces something critical:

Care should not vary from visit to visit.
It should be structured, measurable, and consistent.

Because when you can see what you’re doing, you can refine it, and repeat it.

In this video, Dr. Michael Johnson demonstrates key treatment principles of Cox® Technic while a Palmer student experiences the treatment firsthand.

This isn’t just a movement. There are specific clinical goals behind it.

During flexion-distraction and decompression, the treatment aims to:

🔵 Reduce pressure inside the disc
🔵 Create a negative pressure environment within the disc
🔵 Encourage movement of disc material away from the nerve
🔵 Increase the size of the foraminal space, especially with lateral bending

These mechanical changes are why this technique is often used in cases involving disc herniation, radiculopathy, stenosis, and other nerve compression conditions.

For the patient, the treatment often feels gentle and relieving.
For the provider, the goal is controlled, precise decompression delivered in a repeatable, protocol-driven way.

This is why Cox® Technic is often used for patients who cannot tolerate high-force treatment and for complex spine cases that require a more structured approach.

"I am Cox® Technic Certified, but not listed as ‘Active’. Why? How can I get re-certified and re-listed?"

Once a physician becomes Certified in Cox® Technic, they are listed as Active. To remain Active and listed in the Find a Physician referral directory, doctors must complete a Cox® Technic certification course at least once every two years.

This can include:
- Part I
- Part II
- Part III
- Part IV
- Honors Course
- Or other approved re-certification options

Physicians who have not attended a certification course within the past two years are still Certified, but are listed as Inactive and are not included in the public Find a Physician referral directory.

Why does this matter?
Because Cox® Technic is a protocol-driven system, and ongoing education ensures that physicians stay current with technique updates, clinical protocols, and best practices, especially when treating complex spine cases.

Remaining Active:
➡️ Keeps your skills current
➡️ Keeps you connected to the Cox® Technic community
➡️ Keeps you listed in the referral directory
➡️ Helps patients find you

If you are currently listed as Inactive and would like to be re-certified and re-listed as Active, you can do so by attending an approved Cox® Technic certification course.

Staying active is part of maintaining a structured, clinical, and up-to-date approach to spine care.

Here’s how one provider helped a patient with a disc fragment, a synovial cyst, and severe stenosis, all at the same time.

A 65-year-old patient presented with severe hip and thigh pain, numbness in the shin, and difficulty walking. Her pain was constant and worsening, and everyday activities like standing, walking, and climbing stairs were becoming difficult.

Imaging later revealed severe lumbar degeneration, a free disc fragment, a synovial cyst, and foraminal stenosis compressing the nerve, a complex spine case involving multiple pathologies.

The provider began a structured, non-surgical treatment plan using Cox® Technic flexion-distraction and decompression, combined with supportive therapies and guided exercises. Progress was gradual but consistent.

Over the course of care, her pain levels decreased significantly, her disability score improved, and she was eventually able to walk without assistance.

Cases involving multiple spinal conditions are often the cases that require the most structure, the most precision, and the most patience, from both the provider and the patient.

In spinal care, force is not the goal.
Control is.

One of the key ideas behind flexion-distraction is not just the movement, it’s the control of movement. The goal is to create a controlled environment where the spine can be treated gently, precisely, and repeatedly, without relying on operator strength.

This is where instrument design matters.

With the Cox® Instrument, the physician is not fighting the patient’s body weight, muscle tension, or guarding. The instrument is balanced and tensioned so the caudal section moves smoothly and predictably, allowing the doctor to focus on clinical decision-making, tolerance testing, and protocol, rather than force.

That changes the treatment environment completely.

Instead of:

❌ Forcing movement
❌ Guessing end range
❌ Fighting muscle resistance

The provider can:

✅ Control motion precisely
✅ Adjust tension appropriately
✅ Work within patient tolerance
✅ Deliver repeatable decompression

Because in complex spine cases, precision matters.
And precision requires control.

Back pain can be one of the most frustrating and isolating health challenges a person can face.

Many patients feel like they’ve tried everything, physical therapy, medications, injections and still don’t have answers.

One of the most valuable things a patient can find during that journey is reliable information and real clinical insight.

That’s exactly what The Back Doctors Podcast with Dr. Michael Johnson provides.

With over 300+ episodes, the podcast features conversations with spine specialists from around the world who share real patient cases, treatment approaches, and clinical outcomes. These are not general discussions, they are detailed conversations about complex spine conditions and how providers approach them.

Many episodes focus on patients who were told surgery was their only option, and the non-surgical treatment paths that helped them instead.

Many of the physicians featured on the podcast use Cox® Technic, a protocol-driven, non-surgical spinal decompression approach designed for complex spine conditions. The podcast also provides a Find a Doctor directory, helping patients locate certified Cox® Technic providers.

If you are a patient searching for answers, or a provider who treats complex spine cases, this podcast is a valuable clinical resource and a reminder that there are structured, non-surgical options available for many spine conditions.

What happens when a future provider experiences a different approach to spinal care for the first time?

During a full-spine session, moving from lumbar to thoracic to cervical, a Palmer graduate student experienced flexion distraction and Y-axis techniques with Dr. Michael Johnson in a way that challenged their expectations.

Instead of force, the focus was on controlled movement and decompression.

Instead of quick adjustments, the experience emphasized positioning, motion, and precision.

The result?

✅ A noticeable sense of decompression through the lower back
✅ Improved mobility and rotation,
✅ Relief in areas the patient didn’t even realize needed attention, especially through the cervical spine

For someone early in their clinical journey, the takeaway was clear:

Not every patient responds best to high-velocity approaches.
Sometimes, a more controlled, decompression-based method can create a completely different experience.

And for providers, that opens the door to treating a wider range of patients with more flexibility, more precision, and more intention.

A 36-year-old patient presented with severe low back and left leg pain radiating to the foot with numbness. He had experienced a prior disc issue, improved, and returned to normal activity… but without long-term stabilization, the condition returned more severe.

Imaging confirmed a significant L5-S1 disc herniation along with degenerative changes.

Instead of aggressive or force-based care, the provider followed a Cox® Technic approach:

• tolerance testing to ensure safe starting point
• targeted, controlled spinal decompression
• gradual progression based on patient response

Results:
✅ Pain reduced within days
✅ Symptoms centralized within the first week
✅ Leg pain resolved by day 10
✅ Full remission achieved in ~3 weeks

But the key takeaway wasn’t just recovery, it was understanding that without structure, education, and follow-through, recurrence is likely.

In disc cases, treating the problem is one step.
Managing it long-term is what defines outcomes.
