Diamond Willow Assisted Living: Grand Rapids Here at Diamond Willow we take pride in our person centered care model which is uniquely different from most diagnosis based care systems.

We offer small intimate community settings of 8-16 residents per home and site based RN’s who work with the onsite leadership team and the direct care staff to assure that your loved one’s needs are being met. With that comes the belief that bigger is not always better. Each setting is designed to be intimate with fewer residents per suite, allowing us to have smaller communities to promote our person centered care model in a truly calm and comforting home-like environment. This affords us the ability to provide one on one attention in a more positive, healthy atmosphere to each and every individual. It is because of this model that we offer the industry leading staffing ratios of 1:5 during awake hours and 1:13 overnight. We also recognize that moving multiple times later in life is everything short of desirable. As care needs increase and evolve our person oriented program is designed to provide varying levels of care in a personal suite from arrival through end of life, highly reducing the need to change facilities in the future.

🎄 Attention Grand Rapids Diamond Willow friends and families. We would like to remind you about our annual Christmas party coming up.

All friends and family welcome.

You Matter To Us at Diamond Willow in Grand Rapids

It’s international You Matter to Me Day today. This is a day we are meant to let our loved ones know that they’re important and bring meaning to our lives. It’s a simple gesture, but one we often forget or push off for a later day. The senior citizens in our lives often don’t hear these words enough. Their stories brighten our day and their experiences shaped generations. Without them we wouldn’t be here. I hope you’ll join Diamond Willow Memory Care and Assisted Living in honoring and recognizing our senior loved ones and the impact they continue to have on our lives. They do matter!

Why Diamond Willow?
• We accept MA/Elder waiver and can accept EW in pending status.
• We will provide 2 person transfers and lift assistance.
• We can provide alternative texture diets and thickened liquids.
• We have a step-in whirlpool bathtub in each house.
• We partner with a hair dresser for onsite salon services.
• We are able to utilize bed, chair, and bathroom door alarms to keep frequent fallers safe.
• We allow halo and Q-bar bedrails for transfer assistance.
