⭐️⭐️⭐️⭐️⭐️

"Loved my experience at CPR! I worked with Tom and Maya who are both extremely talented PTs and positively delightful to work with. I came in for a hamstring issue and they had me back to 100% in no time. Very grateful to both them. Highly recommend!"

At The Center, we are now using Limber Health to help track your progress and improve your care.

These short surveys—called outcome measures—give your therapist important information about your pain, mobility, and daily function. Your answers help us better understand how you’re doing and allow us to adjust your treatment plan as needed.

What to expect:
• Each survey is sent by email or text and only takes a few minutes to complete

• Your first survey will be sent about 2 days before your initial appointment, this will take about three minutes

• During your care, you’ll receive a quick check-in every 2 weeks, these take about a minute

• After you are done with therapy, you may receive occasional follow-ups to ensure you are continuing to do well

These tools help us measure your progress over time and make sure your care is moving in the right direction.

Your input matters—it helps guide your care every step of the way.
