Care Resources PACE

Care Resources  PACE For most people, there are little to no deductibles or co-pays. Participants who do not meet PACE eligibility guidelines can pay for services privately.

Care Resources is a Program of All-inclusive Care for the Elderly, or PACE®, that provides wrap-around supportive services for people 55 years and older to keep them healthy and living independently in their homes. We partner with participants and their loved ones to develop a comprehensive care plan that meets the physical, mental, emotional and social needs of each participant and keeps them out of nursing homes. Through our day centers in Grand Rapids and Lowell, we offer:
• Primary health care
• Home health care
• Clinic
• Prescription medication
• Rehabilitation services
• Activities
• Nutrition services
• Social services
• Transportation

PACE uses Medicare and Medicaid funds to cover all medically necessary care and services. Care Resources PACE serves those who are 55 and older and are:
• Residents of Kent County
• Residents of Allegan, Barry, Ionia and Ottawa counties living in these ZIP codes: 48809, 48815, 48846, 48849, 48865, 48881, 48897, 49058, 49302, 49315, 49316, 49323, 49325, 49328, 49331, 49333, 49344, 49348, 49418, 49428, 49435, 49534, 49544

Despite the fact PACE has been around for more than 50 years, this nationwide model of care funded by Medicare and Medicaid remains, unfortunately, a well-kept secret.

We believe that needs to change so eligible older adults in West Michigan can take advantage of the all-inclusive services available to help them age with dignity and where we all want to be – in our own homes.

We'd like to recognize Day Center Personal Care Attendant/Lead Allison VanZandt as our November RISE Above Award recipient! 🎉 She was nominated by her coworkers and selected by the “Recognize, Inspire, Shine, Exceed the Limits” Committee. Here’s what the committee shared:

Allison is an excellent communicator, and she provides excellent care. She assures participants and her fellow team members that she is there to help. One weekend while on call, she offered to go with an aide to a participant’s house after another called in. She never wavered from the care she gave to the participant, changing her weekend schedule to go back a second time to ensure everyone felt comfortable and supported.

Thank you, Allison, for putting the participant’s needs first and supporting your coworkers. We are lucky to have you on our team!
