Meet Anna, an RN Case manager here at Heartlinks. 💜

"My typical work day includes the fortunate privilege of coordinating and managing patients at the end of life," says Anna. "I keep coming to work because the patients we support deserve a consistent and steady presence of people who provide compassion and respectful care, especially when their life may feel uncertain at times."

We envision a future where all phases of life are honored and supported. This includes the veterans who have returned home, their caregivers, and those grieving the loss of a veteran they love. Thank you for your service!

Our adult and pediatric palliative care patients and caregivers meet with their Nurse Practitioner from the comfort of their own homes, thanks to telehealth. We are grateful to our ARNPs who provide medical care, evaluate and write prescriptions, and work alongside the patient's entire medical team to match the goals and priorities of the patient and family.
