Meet Anna, an RN Case manager here at Heartlinks. 💜

"My typical work day includes the fortunate privilege of coordinating and managing patients at the end of life," says Anna. "I keep coming to work because the patients we support deserve a consistent and steady presence of people who provide compassion and respectful care, especially when their life may feel uncertain at times."

The Heartlinks community is an amazing group of supporters, volunteers, and employees. We will spend time this year highlighting several of the wonderful people who make Heartlinks what it is.

Our adult and pediatric palliative care patients and caregivers meet with their Nurse Practitioner from the comfort of their own homes, thanks to telehealth. We are grateful to our ARNPs who provide medical care, evaluate and write prescriptions, and work alongside the patient's entire medical team to match the goals and priorities of the patient and family. We see the whole you.

Palliative care is specialized and supportive medical care designed to provide relief from the symptoms and stress of a serious illness while you continue to seek treatment. It focuses on improving the quality of life for both the individual and their family, helping you feel as good as possible each day.

🎀 Adorn a Headstone
Each year, we have the honor of placing wreaths on the graves of loved ones - a tradition that allows families to see beauty and care woven into their remembrance. If you would like a wreath placed for your loved one, you may purchase one through this fundraiser.

🎄 Decorate Your Home
Our evergreens, chosen for their festive beauty and fragrance, can also embelish your home while supporting Heartlinks’ hospice, palliative care, and grief support programs. Every wreath and evergreen purchased helps us continue to bring compassion, comfort, and quality care to individuals and families during this meaningful time of year.

Order at the link in our bio.

We see the whole you.

Each person’s journey is different, and so is their care. We make sure our care meets more than physical needs, including emotional, cultural, and spiritual needs. That’s what holistic care looks like. Because it’s not just about medicine. It’s about listening, connection, support, and seeing all of who you are.

Celebrate National Hospice & Palliative Care Month with us! Follow along and share your experiences.
