HERE IS WHAT YOU NEED TO KNOW

PBMs, or Pharmacy Benefit Managers, are middlemen in the prescription drug system.
They're companies that work between your insurance company, drug manufacturers, and your local pharmacy. Their job is supposed to be managing drug costs and helping people get their medications.
But here's what's really happening — and how they're putting local independent pharmacies out of business:

1. PBMs Decide What Pharmacies
Get Paid
2. PBMs, or Pharmacy Benefit Managers, are middlemen in the prescription drug system.
They're companies that work between your insurance company, drug manufacturers, and your local pharmacy. Their job is supposed to be managing drug costs and helping people get their medications.
But here's what's really happening — and how they're putting local independent pharmacies out of business:

1. PBMs Decide What Pharmacies
Get Paid, often paying the pharmacy less than what the pharmacy pays for the drug
2. They charge the pharmacy “DIR” fees, these are meaningless and unnecessary fees to help line their pockets
3. They favor their own pharmacies. Yes, they actually own their own pharmacies. (CVS, mail order pharmacies) they do their best to steer patients away from Independent Pharmacies
4. There is no transparency, their contracts with pharmacies are confusing and very unclear when explaining how they will reimburse the pharmacy

The Result?
• Independent pharmacies can’t compete
• They’re losing money on prescriptions
• Many are closing their doors

This means less access to care, especially in rural or underserved areas where the local pharmacy is the only health resource around

The Big 3 PBMs
Three huge companies control 80-90% of the
PBM market in the U.S.:
1. CVS Caremark (owned by CVS Health)
2. Express Scripts (owned by Cigna)
3. OptumRX (owned by UnitedHealth Group)
These parent companies are Fortune 100 giants worth hundreds of billions of dollars:
