✔️ A new lump in the breast or underarm
✔️ Swelling or thickening of part of the breast
✔️ Changes in size, shape, or appearance
✔️ Skin dimpling, redness, or flaking
✔️ Ni**le pain, discharge, or pulling inward

Are you a compassionate, experienced RN with a heart for end-of-life care? We're looking for a dedicated Hospice RN Case Manager to join our Greenfield Hospice Team and make a meaningful difference in the lives of patients and their families.

🌟 What You'll Do: 🌟
-Coordinate and manage individualized care plans for hospice patients
-Provide skilled nursing care, symptom management, and emotional support
-Serve as the primary point of contact for patients, families, and interdisciplinary teams
-Collaborate with social workers, chaplains, and CNAs to ensure compassionate care

What We’re Looking For:
✅ Active RN license (required)
✅ Experience in hospice or palliative care (preferred)
✅ Strong clinical, communication, and critical thinking skills
✅ A heart for compassionate, patient-centered care
