This past fall and winter marked an important step forward for combat casualty care in the Baltic region.

At the request of Frontline Medical Solutions, Ragged Edge Solutions developed and delivered multiple iterations of an original 3-day blood transfusion curriculum for the three Baltic nations, made possible through a grant awarded by the Baltic American Freedom Foundation. The course focused on Tactical Combat Casualty Care principles from point of injury through walking blood bank operations and emergency fresh whole blood transfusion in combat settings—an area of urgent need. While there have been some limited walking blood bank initiatives on deployments and in training, a standardized, Baltic-wide, emergency combat fresh whole blood transfusion policy framework remains a critical gap this training aimed to help address. This program was designed to help jumpstart that effort by building shared knowledge, confidence, and clinical competence.

In October, Ragged Edge Solutions instructors traveled to Lithuania for the first three course iterations. The final two courses were completed this month in Tartu, Estonia. Across all five iterations, 105 students, a mixed cohort of military medics, physicians, nurses, and first responders from conventional units, completed the training.

We are especially grateful that the Ministry of Health of the Republic of Lithuania is currently in the process of accrediting the course, reinforcing its long-term value and sustainability.

CLS is not enough for SOF. The training of non-medics to a higher degree of medical proficiency is an untapped resource to quickly and drastically increase team organic medical capability and capacity. There is no comparison when watching a small SOF team with competent non-medics versus one without... Treatments are more timely, MASCALs are less chaotic, and, when(if?) a Medic/Corpsman is present, they can oversee multiple patients as their non-medic teammates perform many of the functions normally required by a Medic. This gives them space to think and manage. When a team Medic is injured and there is no junior medic, we have seen non-medic, Special Operations Austere Medical Technicians (SO+AMT) step up and provide live saving interventions in real world combat and non-combat situations.

Now Registering: Austere Emergency Care (AEC) – UNC Chapel Hill | Dec 1–4, 2025

We’re heading back to Chapel Hill this December with one of our most in-demand civilian courses — Austere Emergency Care (AEC) — a 4-day, hands-on training event designed to prepare medical providers for when everything goes wrong and help isn’t coming.

Whether you’re a paramedic, flight medic, rural EMS provider, hospital based provider , AEC pushes you out of the classroom and into realistic, high-stress scenarios that test not just your medical skills, but your ability to lead, adapt, and survive in resource-limited environments.
