"What does a plan include? Every patient's plan is different, because it's personalized by their provider and the team. But here are a few pieces every good care plan needs:

1) A list of the medical conditions being addressed by the plan
2) The patient's prognosis and expected outcomes
3) Quantifiable care goals
4) Planned interventions to help patients reach those goals

Helping someone achieve a health goal or just helping them in general to feel better, embrace moments of humanity and community, and discover ways to recover and live their best life!

Let me count the ways....I love so many parts!
