As we head into the new year, we want to make sure your visits go as smoothly as possible. Please keep the following in mind for your upcoming appointments:

✔️ Bring your updated insurance card!
Insurance cards must be scanned every year, so don’t forget to bring yours with you.

✔️ Annual HIPAA form required.
Please arrive a little early to complete your updated HIPAA paperwork for 2025.

✔️ We do NOT accept Medicaid.
If you’re enrolling in a dual plan, double-check that it isn’t paired with Medicaid—we love our patients and would hate to lose you due to a plan change.
