Community Health Alliance - Sojourner Recovery - Transitional Living TLC Our programs include:

Sojourner Recovery Services, an agency of Community Health Alliance, is a full spectrum substance use treatment center.

Community Health Alliance, through Sojourner Recovery Services and Transitional Living-TLC, utilizes individualized treatment programs for general mental health and overcoming the disease of addiction. We have 7 residential facilities spread over Butler, Warren, and Preble County as well as 2 outpatient centers and our medical clinic located in Hamilton Ohio. We specialize in substance use treatment for adult men, adult women, and pregnant or post-partum mothers. When you are referred to Sojourner Recovery for services, you can expect to receive an assessment on your first day either by telehealth or face to face by a certified addiction counselor to determine the appropriate level of care. We offer withdrawal management, residential treatment, partial hospitalization, intensive outpatient, and non-intensive outpatient. Childcare is offered during group time at several of our facilities, and we accept mothers with children under 12 at 2 of our residential locations. Medication Assisted Treatment
Sojourner offers the opportunity to be assessed by a physician for medication assisted treatment. The medications relieve withdrawal symptoms, reduce cravings, and bring about a biochemical balance in the body. We also can treat minor physical health conditions and work closely with several local primary care providers. Perinatal
Sojourner operates a prevention program focused on women of child-bearing age. We utilize a holistic approach to provide services to drug dependent and high risk pregnant and post-partum women before, during, after, or in lieu of treatment in a variety of settings. We strive to help women achieve abstinence and maintain sobriety throughout the last trimester of their pregnancy, thereby delivering a “drug free baby” and improving the quality of life for both mother and infant. Sojourner operates several Recovery Houses in Hamilton, Ohio. We have 3 houses for single men, 1 house for single women, and 1 house for pregnant women with children under 2. We offer sober support, peer support, and a client driven length of stay for everyone who chooses to reside in one of our recovery residences. Discharge Recovery Planning
Sojourner’s discharge planning services start at the point of admission and are an integral part of our recovery programs. We strive to provide clients with the skills and tools necessary to successfully transition once treatment is completed. Focus is placed on developing a recovery action plan which includes, among other things, sober support, housing, medical needs, employment, parenting and education.

On the first Monday of November, Rachel DeWitt, Community Engagement Coordinator at Community Health Alliance, joined a coalition of local providers to go door to door in Middletown, Ohio — meeting neighbors face to face, offering a friendly smile, and sharing essential resources.

This community outreach effort was organized by HopeLine, bringing together agencies united in compassion and purpose. Twice a month, this dedicated group hits the streets, bringing care directly to those who need it most. On this day, they handed out about 50 bags filled with snacks, hygiene items, and resource information from the agencies in attendance.

Together with our partners — New Horizons Centers,
Modern Psychiatry and Wellness of Hamilton, Woodhaven Residential Treatment Center, Full Circle Recovery Services, Haven Behavioral Hospital of Dayton,
and Sunrise Treatment Center —we’re showing what it means to care beyond clinic walls.

With so much happening (or not happening) in Washington, we thought it's worthwhile to share some information regarding WIC benefits as it affects the communities and individuals we serve - and how they are NOT ending! 🛒💛

Even though some SNAP benefits are changing, WIC is a separate program and continues as usual.

Thanks to HopeLine SLIM OF OH INC CareSource Lumiere Healing Centers New Horizons Centers for coming out on a cooler Thursday to celebrate Halloween with the community the best way possible - with some added sugar!

And thanks to First United Methodist Church for hosting the site (and for all the amazing food) for this fun event - it made for some great smiles to about 150 people who came out!

This week, EmPath for Autism had the honor of joining the Butler County Crisis Intervention Committee - where local law enforcement, behavioral health agencies, and community partners came together with one united purpose: to make our community safer and more connected.

Today (October 21), we celebrate one of the most iconic time-travel stories ever told — but it’s also the perfect moment to reflect on our own journeys through time.

In recovery, we often look back at where we’ve been — not to stay there, but to understand how far we’ve come. And just like Marty McFly, we also look ahead — imagining a future where we’re stronger, healthier, and just have it more "figured out."

When we believe in that version of ourselves, it changes the choices we make today. Because no matter what’s behind us, the future is always unwritten — and it’s never too late to shape it - let's shape it together!
