Dear Patients of Victoria Family Practice,

Following the unexpected death of Dr. Timothy Raines, we are sorry to announce that as of April 12th, 2024 Victoria Family Practice has permanently closed.

This letter is to inform you that your medical records have been transferred to Morgan Records Management, LLC . Morgan Records Management is a highly rated medical record custodian and we have had a great experience working with them. We believe that your records will be in good hands with their company.

To obtain a copy of your records, please submit requests to one of the following:

Please Note:

Morgan Record Management will maintain your records in accordance with applicable confidentiality, security, and HIPAA laws. Adult records will be kept for 10 years from the last date of service at from Victoria Family Practice. Records for minors will be kept until the child turns 18 years old.

In accordance with Virginia law, there will be a nominal processing fee charged for each set of medical chart copies.

Patients are allowed to request multiple sets of their records over the retention period. Again, there will be a processing fee charged for each set.

There may be a slight transition period from the time the business is closed to the time Morgan Records Management has full access to your records. You can still sign up to wait in the online queue during this time.

A memorial service will be conducted at 11:00 AM, Friday, March 22, 2024 at Fox Hill Central United Methodist Church with Rev. Eric Vaudt officiating.
Donations may be made in his memory to Fox Hill Central United Methodist Church.

Dr. Timothy Lee Raines of Yorktown, VA passed away suddenly on March 5, 2024. He was preceded in death by his mother, Jean Raines, his father, Walter Pete Raines, and his cherished granddaughter, Delilah. He was sixty-one. A native of Hampton, he was reared in Elizabeth Lakes, where he graduated

The Covid Vaccine is being handled primarily by, and through, the Virginia Department of Health. Please go to their Web Site for the latest information. Dr Raines also suggests that EVERYONE get the Covid Vaccine
