About the Role:
As a CRNP, you’ll provide high-quality primary and preventive health care services, with a focus on serving patients of all income levels, especially the underserved. Working collaboratively with our physicians, you’ll:
Conduct physical exams, obtain medical history, and order/interpret diagnostic tests.

Develop and implement therapeutic care plans.
Administer medications, perform minor procedures, and educate patients on health and illness prevention.
Collaborate with physicians on acute and long-term care needs, ensuring continuity of care.
Maintain patient records, confidentiality, and provide patient/family education.

Qualifications:
BSN required; MSN preferred.
Successful completion of an approved Nurse Practitioner program.
Demonstrated competencies relevant to the CRNP role.

🌟 Welcome Heather Hamme, CRNP! 🌟

We are excited to introduce Heather Hamme, CRNP, as the newest member of our Hanover Family Practice team! Heather brings over 25 years of nursing experience, including critical care, post-acute, and senior care. She holds a master’s degree as a family nurse practitioner and has a strong background in orthopedics, emergency care, and sleep medicine.

Heather began her nursing career in 1993 and has worked in a wide range of healthcare settings—from emergency rooms to private care homes and hospital critical care units. Most recently, she provided care for community residents at Homewood, across all levels of care.

She is passionate about delivering patient-centered care and is experienced in treating conditions such as heart disease, diabetes, asthma, anxiety, and more.
