Addressing mental health early shapes healthier futures. Johns Hopkins Health Plans recently awarded a $10,000 grant to Boys & Girls Clubs of Greater Washington for their Youth Wellness Symposium. The event brings together 100 young people across the Greater Washington region for peer-led discussions and wellness workshops focused on mental health and emotional awareness.

When patients are food insecure leaving the hospital, they’re 18% more likely to be readmitted within 30 days. Johns Hopkins Health Plans awarded a $10,000 grant to the Community Action Council of Howard County to support a program with Johns Hopkins Howard County Medical Center that connects recently discharged patients with same-day grocery delivery. This program provides immediate nutritious food and connects patients with ongoing support through the Howard County Food Bank.

When seniors can live safely in their own homes, they maintain independence, health and connection to their communities. Johns Hopkins Health Plans recently presented a $10,000 grant check to Habitat for Humanity of Wicomico County to support its Aging in Place Program, which is bringing critical safety modifications to 25 households across Wicomico and Somerset counties. Installing features such as walk-in showers, handrails, raised toilets and safer flooring helps seniors avoid the falls and injuries that lead to hospitalization and loss of independence.
