Heart failure is a major national health challenge. It remains the only cardiovascular disease that continues to rise, and it's the number one cause of hospitalization in adults over 65.

During Heart Month, we're highlighting our Heart Failure Empowerment Program designed for our home health patients. We help patients understand their disease, recognize early warning signs and adopt heart-healthy habits. We just finished Nurse Appreciation Week and we showered them with gifts and treats but we will continue to celebrate them daily. There is no replacing what you sacrifice of yourselves to go above and beyond the call of duty.
