L UPDATE

To My Dear Patients,

The office is now permanently closed. I have retired from the clinical practice of Ophthalmology.

Your medical records have been packed up and have been transferred to Memorial Eye Institute (MEI), 4100 Linglestown Rd, Harrisburg, PA 17112.
Please know that all your medical records will remain safely on file and confidential through the transfer as per state and federal law. MEI will serve as the legal custodian of your medical records. While you do have a choice in who will be your new eye care provider, I am confident that the team at MEI together with the addition of Dr Stephanie Arlotti will provide you with excellent patient care. Dr Arlotti will start seeing patients at MEI in October. You can call and schedule an appointment with her at (717) 657-2020.

If you decide to transfer to a new eyecare provider outside of their practice, you will need to contact MEI, (717) 657-2020, to have your records transferred.

If you have any eye issues after August 15, 2024 you will need to call the MEI team at (717)657-2020 or our main phone number (717)652-4924 should also put you in direct contact with MEI or go to Community General and see the ophthalmologist on call.

Any appointments you may have had at my office scheduled for after August 15, 2024 will NOT automatically transfer to MEI. You will need to call MEI and schedule a new appointment.

Once again thank you for trusting me with your eyes. It has truly been and honor and privilege to provide you care. I wish you all the best in the future.

Memorial Eye will be the custodian of your chart. Your chart will automatically transfer to their office.

Please wait until after August 19 to schedule your appointment to allow the transition to occur and our phone number to be transferred (717) 652-4924.
